Когнитивно-поведенческий подход в психологической коррекции эмоционального состояния и поведения при ожоговой болезни

Аннотация

Обследовано 50 человек с диагнозом «Ожоговая болезнь II-III степени, нарушение психической адаптации». На диагностическом этапе были использованы методы: клиническая беседа и наблюдение, интегративный тест тревожности, метод рисуночной фрустрации С. Розенцвейга, копинг-тест Р. Лазаруса.

У пациентов с ожоговой болезнью в остром периоде отмечается высокий уровень ситуативной тревожности в виде эмоционального дискомфорта, тревожной оценки перспективы и астенического компонента, повышен удельный вес эго-защитных реакций и показатель чистой агрессии. Отмечаются высокие параметры суммарных самозащитных реакций, высокие значения индекса направленности агрессии, снижение индекса трансформации агрессии и индекса решения проблем, отмечается достоверно высокая «внешняя агрессия» и низкая групповая адаптация с преобладанием копинг-стратегии «дистанцирование».

Программа психологической коррекции по методу когнитивно-поведенческой терапии позволила снизить уровень ситуативной тревожности, а также возросло использование копинг-стратегии «самоконтроль».

Введение

Ожоги представляют серьезную проблему в области здравоохранения, включая длительную госпитализацию, обезображивание и инвалидность, часто сопровождаются стигматизацией и неприятием, в связи с чем вопросы предоставления качественной психологической помощи пострадавшим остаются весьма актуальными [6]. Угроза жизни и здоровью, длительная госпитализация, выраженный болевой синдром, риск инвалидизации и получения физических уродств — пациент с ожогами страдает от непрерывного травматического стресса, что может стать причиной психологической дезадаптации [17; 22].

У пациентов с ожоговой травмой серьезно снижается качество жизни [14]. Оказание психологической помощи пациентам с ожоговой болезнью способствует повышению эффективности проводимого лечения, стабилизирует эмоциональную сферу, актуализирует необходимые для реабилитации личностные ресурсы [12].

Цель исследования: определить особенности психической адаптации пациентов в остром периоде ожоговой болезни и оказать психологическую помощь на госпитальном этапе клинико-психологического сопровождения.

Объект исследования: эмоциональное состояние, особенности поведения пациента с ожоговой болезнью.

Предмет исследования: тип и направление эмоционального реагирования на фрустрацию, уровень тревожности, стратегии совладания со стрессом и социальная адаптация пациентов с ожоговой болезнью.

Материалы и методы исследования

Исследование проводилось на базе ожогового центра Городской клинической больницы № 1 имени Н.И. Пирогова. В нем приняли участие 50 человек в возрасте от 22 до 35 лет (мужчин и женщин поровну) с диагнозом «Ожоговая болезнь II-III степени. Нарушение психической адаптации».

Методы исследования: клиническая беседа и наблюдение; метод рисуночной фрустрации С. Розенцвейга; интегративный тест тревожности; копинг-тест Р. Лазаруса.

Для количественного анализа данных были выбраны: угловое преобразование Фишера, параметрический t-критерий Стьюдента и коэффициент ранговой корреляции rs Спирмена. Статистический анализ проведен по программе Statistica 10.0 и MS Excel (из состава офисного пакета Microsoft Office версии XP Home Edition).

Результаты исследования и их обсуждение

У пациентов с ожоговой болезнью в остром периоде диагностируется высокий уровень «ситуативной» тревожности (таблица 1) в виде «эмоционального дискомфорта», «тревожной оценки перспективы» и «астенического компонента». Можно предположить, что ситуативная тревожность — это эмоциональный предиктор психоэмоционального реагирования в болезни, влияющий на психическую адаптацию у данного контингента больных. Психологические параметры общей «личностной» тревожности находятся в пределах нормы реакции, что говорит о внутриличностной психической адаптации этих пациентов.

По данным теста С. Розенцвейга, у пациентов с ожоговой болезнью в начале лечения повышен удельный вес эго-защитных реакций (E−D>N−P>O−D), то есть в ситуации фрустрации отмечается склонность к эго-защитным реакциям в форме агрессии, направленной на окружающих, и порицания партнёров по ситуации (E>M>I). Значительно повышен показатель «чистой агрессии» — (e−Е), имеются достоверно значимые высокие параметры суммарных самозащитных реакций (Е+I), (р≤0,05), высокие значения «индекса направленности агрессии» (∑E/∑I). Также наблюдается снижение «индекса трансформации агрессии»: (E/e) — плохо управляемое поведение на стресс, только за счет направления агрессии вовне, показывающее неадаптивное поведение по сравнению со стандартом, «индекс решения проблем» (i/e) снижен, отмечается достоверно высокая «внешняя агрессия» (∑E/∑M) и низкая групповая адаптация (GCR). Эмоциональное реагирование на фрустрацию отличается от стандарта по Тарабриной (таблица 2).

Современный подход к профилактике отдаленных негативных последствий ожоговой травмы предполагает необходимость проведения как можно более ранней психологической реабилитации [9; 10; 15]. Общая задача, которую решает психолог на этапе стационарного лечения, — сформировать ресурсное психическое состояние, основанное на индивидуальной норме, в котором пациент может справляться с трудной жизненной ситуацией ожоговой болезни (с учетом ограничений, накладываемых болезнью). При этом решаются две частные задачи: адаптация к реальности и адаптация к самому себе в новых условиях жизнедеятельности [11].

С целью повышения адаптационного потенциала пациентов с ожоговой болезнью была создана программа психологической помощи, опирающаяся на техники когнитивно-поведенческой психотерапии (КПП) [1; 2; 3; 4; 5].

По мнению Фаустовой А.Г. и Яковлевой Н.В., программа психологического сопровождения пациентов с термическими поражениями должна включать следующие блоки: диагностический, базовый (психокоррекция типичных нарушений, проявляющихся у пациентов ожогового центра); специализированный (психокоррекция нарушений, специфика которых обусловлена локализацией и характером термической травмы); контрольный [12].

У пациентов с ожоговой болезнью выявлено различное стресс-совладающее поведение с преобладанием дезадаптивных копинг-стратегий: «дистанцирование», проявляющее себя в виде отдаления от ситуации, уменьшения её значимости и игнорирования (таблица 3).

Программа психологического сопровождения включала в себя: диагностику факторов, участвующих в психической адаптации при ожоговой болезни; обучение техникам снятия болевого синдрома; снижение уровня тревожности и стабилизацию эмоционального фона; формирование адекватного отношения в болезни; обучение стратегиям совладания со стрессом и социальную поддержку.

Основной этап КПП-коррекции проводился в госпитальных условиях (10 занятий, 1,0 час каждое, 3 занятия в неделю). Поведение и эмоции при ожоговой болезни обусловлены не столько внешним воздействием, сколько в дальнейшем — их когнитивной интерпретацией и поведением, которое может быть следствием дезадаптивных и дисфункциональных автоматических мыслей, установок и других показателей [2; 5].

Особое место в структуре переживаний пациента занимает болевой синдром, выраженность которого может привести к развитию у него тревожных, депрессивных или дисфорических реакций [6]. Выделяют 2 типа болевого синдрома при ожоговой травме. Процессуальная боль (острая и кратковременная, но большой интенсивности) — это боль во время или после ухода за раной, в процессе перевязки, ЛФК или других процедур. Фоновая боль (тупая, непрерывная и меньшей интенсивности) — это боль, испытываемая пациентом все время в состоянии покоя. Тем не менее, уровень этой боли следует держать под контролем, так как она увеличивает беспокойство пациентов, особенно по поводу процедур [13; 19]. Боль при ожогах не связана с площадью обожженной поверхности и имеет, по-видимому, во многом эмоциональное, а не физическое происхождение [6; 8].

Психологическая работа с болевым синдромом основывалась на техниках когнитивно-поведенческой терапии [3; 21] и проходила в несколько этапов: информирование пациента о взаимовлиянии болевого синдрома и эмоционального состояния; переоценка значимости болевых симптомов; коррекция поведения путем переключения внимания на эмоционально-положительные стимулы, с активизацией деятельности, доставляющей удовольствие. Все пациенты обучались аутогенной тренировке с использованием музыкотерапии, дыхательным техникам и когнитивным техникам работы с болью посредством ее созерцания, ощущения, мысленного раскладывания на компоненты, смещения фокуса на здоровые участки тела, трансформации ощущения боли в детальный визуальный образ с последующей редукцией.

В зависимости от стадии заболевания психологическое сопровождение пациентов с ожоговой болезнью имеет свои особенности. На стадии ожогового шока основными факторами стресса являются внезапность, боль, страх, стыд. У пациента актуализирована потребность в безопасности. Для облегчения состояния пациента использовались техники «насыщения информацией», «когнитивного структурирования», прием «вентиляции эмоций». Большое внимание уделяется созданию рабочего альянса между пациентом, его родственниками, медицинским персоналом и психологом [16; 18; 20].

На стадии токсемии пациент испытывает оторванность от привычного социального окружения, недостаток эмоциональной поддержки, страдает от тревожных мыслей о своем будущем, переживает изменения, связанные со своим внешним обликом. Для облегчения эмоционального состояния использовались приемы: техника «вентиляции эмоций», методика «инверсии возможностей», прием «когнитивной переработки» и «структурирования», десенсибилизация.

У пациентов с ожоговой болезнью были выделены следующие иррациональные установки, приводящие к эмоциональным нарушениям: произвольные умозаключения, избирательное невнимание; чрезмерные обобщения, преувеличения или преуменьшения; неспособность взглянуть на себя со стороны (пациент требует от себя невозможного); дихотомическое мышление «черно-белые суждения» (все или ничего, хорошо или плохо); высокое долженствование.

На стадии реконвалесценции пациент проявлял обеспокоенность своим будущим,  возможными  проблемами  в  профессиональной  и  межличностной сфере. «…массированный стресс во время ожога разрушает сложившиеся взаимодействия между индивидуальными особенностями разных уровней…, что оказывает отрицательное влияние на развитие болезни и компенсацию дефекта. Однако преодоление психологических последствий травмы может вывести человека на новый … уровень функционирования. Меняясь, человек проходит через спектр разнообразных возможностей, обретая в итоге свою целостность» [7].

На данном этапе проводилась работа с чувством беспомощности и безнадежности, подготовка больного к новой социальной ситуации жизни после лечения ожога и к принятию себя. Проводилось исправление когнитивных ошибок, имеющихся у больного, использовались приемы разъяснения — логический, эмпирический и прагматический диспут, сократовский диалог, а также техники «насыщение информацией», «отдаление», «эмпирическая проверка», «методика переоценки», «декатастрофизация», «использование воображения», «переоценка ценностей», техника «мобилизация надежды».

После проведенной программы психологической коррекции выявлено, что уровень ситуативной тревожности достоверно снизился практически по всем показателям: достоверное повышение общей личностной тревожности с низкого до нормального уровня в основном за счет показателя «социальные реакции защиты», что свидетельствует в пользу повышения психической адаптации данного контингента больных.

По результатам теста С. Розенцвейга значение эго-защитных реакций (E−D>N−P>O−D) трансформировалось за счёт увеличения реакций импунитивной направленности (M>E>I), то есть в ситуации фрустрации повысилась склонность к эго-защитным реакциям в форме оправдания (снятие вины с партнёров по ситуации). На втором месте стоят реакции с фиксацией на разрешении ситуации (N−P) за счёт принятия на себя ответственности за удовлетворение ситуативно возникающей потребности или решение проблемы. Можно отметить достоверное снижение экстрапунитивных реакций и повышение показателя импунитивных, безобвинительных реакций, достоверно снижение индексов агрессивного реагирования и повышение групповой адаптация (GCR). Эти данные говорят об улучшении психической адаптации обследуемых пациентов с ожоговой болезнью.

Сравнительный анализ результатов стресс-совладающего поведения показал, что в результате КПП-коррекции достоверно увеличилась частота использования копинга «самоконтроль» и снизилась вероятность использования копингов «дистанцирование» и «избегание».

Таким образом, в процессе КПП-коррекции повышается психическая адаптация пациентов с ожоговой болезнью, снижается тревожность и стабилизируется эмоциональное состояние, формируются более адаптивные копинг-стратегии, которые могут использоваться как психический ресурс на этапе выздоровления.

Выводы

1.

Установлены особенности психической адаптации при ожоговой болезни: высокий уровень ситуативной тревожности, дезадаптивные копинг-стратегии «дистанцирование» и «избегание», низкая фрустрационная толерантость, высокие показатели внешне агрессивного поведения и низкий уровень социальной адаптации.

2.

В процессе КПП-коррекции пациента с ожоговой болезнью формируются адаптивные копинг-стратегии, которые могут использоваться пациентом как психический ресурс на этапе выздоровления: высокий самоконтроль, высокий уровень социальной поддержки.

Cognitive-behavioural approach in the psychological correction of the emotional state and behavior in burn disease

Abstract

Were examined 50 persons with a diagnosis of burn disease II-III degree. In the diagnostic phase were used clinical interview and observation, the integrative test of anxiety, S. Rozenzweig picture frustration study, coping test R. Lazarus.

Patients with burn disease demonstrated a high level of "situational" anxiety (emotional discomfort, anxiety rating perspectives and asthenic component). Significantly increased net aggression, high parameters of the total self-shielding of the reactions, high values of the index of directionality of aggression, reduced transformation index of aggression and index to solve problems, noted significantly higher "external aggression" and the low group adaptation. According to the coping test identified stress coulduse behavior with a predominance of non-adaptive coping strategy "distancing".

Then was formed by a program of psychological support that includes diagnosis of factors involved in adaptation in burn disease; learning the techniques of attenuation of the pain syndrome; reduction of anxiety and stabilization of the emotional background, the formation of an adequate attitude to the disease, training of coping strategies with stress and social support. The program was a technique used in cognitive-behavioral therapy.

After the program of psychological correction the level of "situational" anxiety significantly decreased for almost all indicators. The results of the coping test showed that a significantly increased frequency of use of coping self-control.

Introduction

Burns are a global problem in healthcare. Not fatal burns are a major cause of morbidity, including prolonged hospitalization, disfigurement and disability, often with resulting stigma and rejection, in connection with the provision of quality psychological assistance to victims remains very relevant [7]. The threat to life and health, long-term hospitalization, severe pain, the risk of disability and a physical deformity — burn patient suffers from continuous traumatic stress that may cause psychological maladjustment [16; 20]. In patients with burn injury seriously affects the quality of life [14]. The provision of psychological care to patients with burn disease enhances the effectiveness of the treatment, stabilizes the emotional field, updates necessary for the rehabilitation personal resources [3].

The purpose of the study: to determine peculiarities of mental adaptation of patients in the acute period of burn disease and to provide psychological support during hospital stay clinical and psychological support.

The object of the research: the online personality of patient with burn disease. Subject of research: the type and direction of emotional response to frustration, level of anxiety, coping strategies with stress and social adaptation of patients with burn disease.

The materials and methods

The study was conducted in the burn center of City clinical hospital № 1 named after N.I. Pirogov. It was attended by 50 people aged from 22 to 35 years (men and women equally) are diagnosed with burn disease II-III degree.

Research methods: clinical interview and observation; S. Rozenzweig picture frustration study; integrative test anxiety; coping test R. Lazarus. For the quantitative data analysis were chosen: corner Fisher transformation, parametric t-student test and the rank correlation coefficient rs Spearman. Statistical calculations were performed in programs, Statistica and MS Excel 10.0 (part of office package Microsoft Office XP Home Edition).

The results of the study and their discussion

Patients with burn disease in the acute phase diagnosed by a high level of "situational" anxiety (table 1) in the form of "emotional discomfort", "anxiety rating perspectives" and "asthenic component". We can assume that the situational anxiety is a kind of emotional predictor of emotional response in diseases that affect psychological adaptation in this population of patients. Psychological parameters the overall "personality" of anxiety are within the norm of reaction, that tells about intrapersonal psychological adaptation of these patients.

According to the S. Rozenzweig picture frustration study in patients with burn disease at the beginning of treatment increased the proportion of ego-defense reactions (E−D>N−P>O−D), that is, in a situation of frustration marked tendency to ego-protective reactions in the form of aggression directed at others and blame partners on the situation (E>M>I). Significantly increased the index of "pure aggression" — (e−E), there are high options total defensive reactions (E+I), the high values of the index of directionality of aggression (∑E⁄∑(I); the decline in the index of transformation of aggression (E⁄e) — poorly controlled behavior to stress, only due to the direction of aggression to the outside, showing maladaptive behavior in comparison with the standard, index of solving problems (i⁄e) reduced significant high notes of "external aggression" (∑E⁄∑M) and low group adaptation (GCR). Emotional response to frustration is different from the standard (table 2).

In patients with burn disease revealed a variety of stress-coulduse behavior, with a predominance of non-adaptive coping strategies "distancing", manifesting itself in the form of distancing from the situation, reducing its significance and neglect (table 3).

Modern approach in the prevention of the negative remote consequences of burn injuries suggests the need for early psychological rehabilitation [10; 11; 21]. A common task that solves a psychologist at the stage of inpatient treatment is to form the mental state of the resource, based on the individual normal, which the patient can cope with his life (taking into account the restrictions imposed by the disease). In this case, solve two particular problems: adaptation to reality and to adapt himself to new conditions of life [12].

To increase the adaptive capacity of the patients with burn disease was established the program of psychological assistance, based on techniques of cognitive-behavioral psychotherapy [1; 2; 4; 5; 6]. According to Faustovа A.G., Yakovleva N.V. the program of psychological support for patients with thermal lesions should include the following blocks: diagnostic, basic (psychological correction of typical violations, manifested in patients of the burn center); specialized (psychological correction of disorders, the specificity of which is due to the location and nature of the thermal injury); control [3].

The program of psychological support included diagnosis of factors involved in adaptation in burn disease; learning the techniques of attenuation of the pain syndrome; reduction of anxiety and stabilization of the emotional background, the formation of an adequate attitude to the disease, learning the strategies of coping with stress and social support. The main stage of the transmission correction was carried out in hospital conditions (10 lessons) 1.0 hours, 3 classes per week.

Behavior and emotions in burn disease is not caused by external influence, as in their further cognitive interpretation and behavior, which can be the consequence of non-adaptive and dysfunctional automatic thoughts, attitudes, and other indicators [1; 4].

In patients with burn disease, the following were highlighted as irrational installation leading to emotional disorders: arbitrary inferences, selective inattention; overgeneralization, exaggeration, or understatement; the inability to look at ourselves (the patient requires the impossible from yourself); dichotomous thinking "black and white" judgments (all or nothing, good or bad); high prescriptions.

Depending on the stage of the disease psychological support of patients with burn disease has its own characteristics. At the stage of burn shock of the main stress factors are suddenness, pain, fear, shame. The patient has actualized the need for security. To facilitate the patient's condition was used the technique of "saturation of information", "cognitive structuring", "ventilation of emotions". Much attention is paid to creating a working Alliance between the patient, their relatives, medical staff and a psychologist [15; 17; 22].

At the stage of toxemia the patient was in isolation from the usual social environment, lack of emotional support, suffering from disturbing thoughts about your future, is experiencing changes associated with their appearance. To facilitate the emotional state of the used techniques: the technique of "ventilation of emotions", the technique of inversion of the opportunities, reception "cognitive processing" and "structuring", desensitization.

A special place in the structure of the patient is a pain syndrome, the severity of which can lead to the development of anxious, depressive or dysphoric reactions. Pain relief for burns not associated with the area of burnt surface and has, apparently, largely emotional rather than physical origin [7]. There may be 2 types of pain in burn injury. Procedural pain (acute and of short duration, but high intensity) is pain during or after wound care, bandaging, physical therapy or other treatments. Background pain (dull, continuous and lower intensity) is the pain experienced by the patient is at rest. However, the level of this pain should be kept under control as it increases the anxiety of patients, especially about procedures [13; 19]. Pain relief for burns not associated with the area of burnt surface and has, apparently, more emotional rather than physical origin [7; 9].

Psychological work with pain syndrome was based on the techniques of cognitive-behavioral therapy [5; 18] and took place in several stages: informing the patient about the interaction of pain and emotional States; a revaluation of the significance of pain symptoms; correction, by switching attention to positive emotional stimuli, with the increased activity pleasurable. All patients were taught autogenic training with the use of music therapy, breathing techniques and cognitive techniques of working with pain through her contemplation, of feelings, of mental unfolding on components, focus on the healthy parts of the body, the transformation of pain into a detailed visual image with the subsequent reduction.

At the stage of convalescence the patient showed concern about their future, potential problems in professional and personal sphere. "…a massive stress during the burn destroys the current interaction between the individual characteristics of different levels … that has a negative impact on the development of the disease and compensation for the defect. However, overcoming the psychological consequences of injury can take a person to new … level of functioning. Changing, people goes through a variety of possibilities, finding in the end his integrity" [8].

At this stage, work was carried out with a sense of helplessness and hopelessness, the patient's preparation to the new social situation of life after burn treatment and acceptance of oneself. Was conducted correction of cognitive errors, available patient, used the techniques of explanation — logical, empirical, and pragmatic disputation, Socratic dialogue, and the techniques of "saturation of information", "distance", empirical testing, methods of revaluation, "decatastrophize", use of imagination, the revaluation of values, the technique of "mobilizing hope".

After the program of psychological correction revealed that the level of "situational" anxiety significantly decreased for almost all indicators: a significant increase in the overall "personality" of anxiety with low to normal levels mainly due to the increased "social reaction" of protection, which is evidence in favor of increasing the mental adaptation of the patients.

According to the results of the S. Rozenzweig picture frustration study value ego-defense reactions (E−D>N−P>O−D) transformed due to the increase in impunitive reactions (M>E>I), that is, in a situation of frustration increased tendency to ego-protective reactions in the form of justification (removing the guilt from the partners on the situation). In second place are the reaction with fixation on the resolution of the situation (N−P) due to the assumption of responsibility for meeting situational needs as they arise or solving the problem. It is possible to note a significant decrease extrapunitive reactions and increasing the level impunitive reactions, significantly the growth index aggressive response and improving the group adaptation (GCR). These data suggest that the improvement of mental adaptation of the patients with burn disease

Comparative analysis of the results of stress-coping behavior showed that as a result of transmission-correction significantly increased the frequency of using coping "self-control" and decreased the likelihood of using coping skills "distancing" and "avoidance".

Thus, in the process of transmission-correction increases mental adaptation of patients with burn disease, reduced anxiety and stabilize emotional state, more adaptive coping strategies that can be used as a mental resource at the stage of recovery.

Conclusions:

1.

The peculiarities of mental adaptation in burn disease: high level of anxiety, non-adaptive coping strategies distancing and avoidance, a low frustration tolerant does, superficially high rates of aggressive behavior and low level of social adaptation.

2.

In the process of transmission of the correction of the patient with burn disease formed adaptive coping strategies that can be used by the patient as a mental resource at the stage of recovery: high self control, high levels of social support.
