Половые различия в эмоциональном реагировании на процедурную боль у пациентов с бытовой ожоговой травмой

Аннотация. Постановка проблемы. Феномену боли при различных заболеваниях посвящено огромное число публикаций, однако, как показал литературный поиск, боль при ожоговой болезни оказалась мало изученной. Отсутствуют сведения об особенностях реагирования на процедурную боль у больных с бытовой ожоговой травмой с точки зрения половых и гендерных различий. Цель настоящего исследования — изучение эмоционального реагирования на процедурную боль, возникающую во время процедур перевязок у мужчин и женщин — пациентов ожогового отделения. Материалы и методы. Обследовано 60 стационарных больных с бытовой ожоговой травмой (30 мужчин и 30 женщин). В работе были использованы методы опроса, наблюдение, вербально-цветовая шкала интенсивности боли из Многомерного вербально-цветового болевого теста. Статистическая обработка данных осуществлялась с помощью критерия χ² Пирсона. Результаты исследования. Опрос медперсонала показал, что женщины-пациентки выносливее переносят боль во время перевязки. Мужчины реагируют агрессивно, несдержанно, злобно, не могут терпеть боли во время перевязок, способны нецензурно выражаться в адрес медперсонала, пытаются обвинить и даже ударить, проявляют безответственность, циничность, неуважительное отношение к медицинским работникам. Женщины, наоборот, более терпеливы и молчаливы, склонны жалеть себя и вызывать сочувствие со стороны персонала. Получены качественные различия в способах эмоционального реагирования на боль у мужчин и женщин — пациентов ожогового отделения. Частотный статистический анализ обнаружил достоверные различия в выборе и количестве способов вербального реагирования на боль между мужчинами и женщинами (χ²=78,09; р<0,01), а также невербально-эмоциональном реагировании (χ²=99,34; р<0,01). Сходство вербальных реакций на боль проявлялось в неконтролируемых реакциях — стоне, криках, вздохах, междометиях. Женщины, в сравнении с мужчинами, в 10 раз чаще молчали и в 10 раз реже употребляли нецензурные выражения, использовали «сдерживающие» стратегии — мимикой, жестами давали понять медперсоналу о силе переживаемой боли. Мужчины чаще переходили на крик, проявляли избыточную и неуместную двигательную активность, что затрудняло проведение перевязочных мероприятий. Использование вербально-цветовой шкалы показало, что 1/3 женщин оценивают процедурную боль как слабую и средней силы, в то время как аналогичное число мужчин определяли боль как очень сильную и невыносимую. Сходные данные получены и при сравнении болевых реакций у пациентов обоего пола с одинаковой степенью ожога III АБ (χ²=7,8; р<0,05). Получены статистически значимые различия по полу при выборе слов-дескрипторов эмоциональных реакций на процедурную боль в общей выборке пациентов (χ²=31,98; р<0,01) и в группе с одинаковой степенью ожога — III АБ (χ²=54,30; р<0,01). Большинство мужчин в оценке своей боли во время процедур перевязок указали на такие эмоции, как злость и гнев, а женщины — страдание и тревогу. Таким образом, анализ данных о самооценке интенсивности процедурной боли у ожоговых пациентов позволил отметить значительные половые различия. Главный вывод заключается в том, что женщины более терпеливо и стоически переносят болевые ощущения при перевязках, что противоречит общепринятым представлениям о том, что женщины более чувствительны к боли, труднее ее переносят. Заключение. Особенности болевого реагирования у пациентов с бытовой ожоговой травмой следует рассматривать в контексте общего состояния организма, клинической картины болезни, переживаемого личностью посттравматического стресса, опыта общения с болью, преморбидных особенностей личности. Ставится вопрос о необходимости проведения сравнительных клинико-психологических исследований больных с бытовой, производственной и военной ожоговой травмой, а также жертв стихийных бедствий и катастроф, сопровождающихся термическими повреждениями.

Проблема боли и обезболивания является центральной в клинической медицине. В настоящее время накоплен огромный материал о проявлениях транзиторной, острой и хронической боли при различных заболеваниях, патофизиологических механизмах боли, лекарственных, физиотерапевтических, психологических и иных немедикаментозных способах ее купирования.

Сила и характер болевых ощущений являются результатом работы двух систем организма: болевой (ноцицептивной) и антиболевой, обезболивающей (антиноцицептивной). Их взаимодействие друг с другом определяет, какие именно болевые ощущения будет испытывать человек [4; 7; 13]. В соответствии с видами болевой чувствительности различают эпикритическую и протопатическую боль. Эпикритическая («первая», «быстрая», «предупредительная») боль возникает в результате воздействия раздражителей малой и средней силы. Протопатическая («древняя», «медленная», «тягостная») боль возникает под действием сильных, «разрушительных», «масштабных» раздражителей.

Международная Ассоциация по изучению боли (IASP) дает следующее определение боли: «Боль — неприятное сенсорное и эмоциональное переживание, связанное с истинным или потенциальным повреждением ткани или описываемое в терминах такого повреждения» [44; 50]. Данное определение указывает, что боль не всегда возникает вследствие повреждения тканей, как это бывает при травмах, воспалительных или дегенеративных процессах, вследствие ишемии, но и также в силу психологических причин, что подчеркивает значение личностно-субъективного фактора. Согласно теории воротного контроля, психосоциальные и поведенческие факторы интегрируются в физиологические процессы субъективного переживания (восприятия, ощущения) боли. Именно субъективность ощущения боли означает, что индивидуальные установки, оценки и копинг-стратегии вносят свой вклад в сохранение, ухудшение или ослабление болевого ощущения или болевого поведения [48; 49]. Болевой компонент, наряду с эмоциональной, интеллектуальной и волевой сторонами, является составляющей внутренней картины болезни (ВКБ), представляя уровень ощущений (чувственный уровень) [21; 30]. При этом именно субъективные ощущения являются основным источником и индикатором изменений в структуре ВКБ [29].

Исследование боли является сложным комплексом проблем. Так, пациенты по-разному сообщают о локализации, характере и продолжительности боли. Различия между мужчинами и женщинами в ответе на боль были подтверждены многими эпидемиологическими, клиническими и экспериментальными данными. Как указывает А.М. Вейн, «боль — печальная привилегия женщин, хотя иногда имеются обратные соотношения» [9]. Они более чувствительны к боли и чаще, чем мужчины, страдают болевыми синдромами [12; 39; 43; 57].

В большинстве случаев обнаруживается, что женщины отмечают боль в большей степени, чем мужчины [11; 44]. Такие же закономерности наблюдаются и в детском возрасте между девочками и мальчиками [46; 47]. Установлено также, что мужчины и женщины сильно различаются в отношении болевого ожидания. В соответствии с гендерными стереотипами (нормами), представители обоих полов считают, что мужчины менее чувствительны к боли, лучше переносят боль и менее охотно отчитываются о боли [52].

В большинстве работ, касающихся проблемы боли, в том числе опубликованных в специализированных отечественных и зарубежных журналах, приводятся данные преимущественно о хроническом ее варианте (головные, спинальные, мигренозные, абдоминальные и др. боли). Было показано, что существуют различия в контроле боли у мужчин и женщин: мужчины в большей степени склонны верить, что боль можно контролировать и демонстрируют низкий уровень катастрофизации (чувство отсутствия контроля над ситуацией, чрезмерные волнения (переживания) по поводу будущего) [54; 55]. По данным одних авторов женщины тяжелее переживают послеоперационные [38] и процедурные боли [53], по другим — не было выявлено достоверных различий в восприятии острой боли [41; 42].

Г.А. Адашинская, Е.Е. Мейзеров, С.Н. Ениколопов исследовали различия в восприятии боли у 41 мужчины и 41 женщины с нейрогенным, соматогенным и психогенным болевым синдромом, и установили, что женщины по сравнению с мужчинами более чем в два раза чаще испытывают сильные боли независимо от формы болевого синдрома, для них характерно выраженное аффективное переживание боли; мужчины ярче выражают эмоциональное отношение при «средних» и «сильных» болях, при болях высокой интенсивности («очень сильной», «невыносимой») им свойственна сдержанность в ее эмоциональной оценке; женщины при оценке боли выбирают большее, чем мужчины число сенсорных и эмоциональных дескрипторов независимо от формы болевого синдрома; пациенты с психогенным болевым синдромом (мужчины и женщины) при характеристике болевых ощущений и эмоционально-аффективного отношения к боли используют большее число дескрипторов по сравнению с другими пациентами [2].

Доказана взаимосвязь между болью и аффективными ответами на боль. Отрицательные эмоции сами по себе усиливают соматическую чувствительность, могут вызывать боль или быть результатом хронической боли [22; 51]. W.E. Haley, J.A. Turner & J.M. Romano показали, что депрессия в значительной степени связана с болью у женщин, в то время как у мужчин депрессия обнаруживает связь с уровнем активности, но не болью [45]. Эти и другие работы указывают на высокую значимость исследований в области гендерной альгологии, однако детальный анализ всех ее аспектов не входит в наши задачи.

Изучение проблемы боли крайне значимо для комбустиологии, поскольку все виды ожогов сопровождаются болью. Боль сопровождает ожоги с первой минуты и практически до полного заживления ран. Острая эпикритическая боль у ожогового больного возникает в момент получения термической травмы и при проведении операций и болезненных манипуляций. На протяжении ожоговой болезни у больного присутствует протопатическая боль, выраженность которой зависит от адекватности проведенного обезболивания и интенсивной терапии в момент травмы, при последующих оперативных вмешательствах и болезненных манипуляциях. Интенсивность болевых ощущений у больных с ожогами зависит от глубины и локализации поражения, фазы раневого процесса. Лидирующие позиции в структуре ожоговой травмы в настоящее время занимает бытовая (свыше 75%) ожоговая травма [23].

Термическая травма вызывает комплекс изменений, проявляющихся эмоциональными, мнестическими, психомоторными и другими видами нарушений. Наблюдения показывают, что психологическое состояние ожогового больного формируется в процессе проведения оперативных вмешательств и выполнения перевязок. В отечественной литературе достаточно полно описаны психопатологические нарушения у больных с ожогами [6; 10; 16; 33; 34; 35], психологические особенности травматического стресса при ожоговой болезни, его динамика и последствия [8; 12; 15; 17; 18; 19; 25; 28; 35; 36] (и др.). Однако, как показали С.Г. Лафи и Н.М. Лафи, сам феномен боли при ожоговой болезни удивительным образом оказался вне поля зрения исследователей [20]. Проведенный авторами литературный поиск позволил им найти лишь одно сообщение по болевому синдрому у обожженных на русском языке, представленное украинскими специалистами [24]. С этих позиций заявленная нами тема изучения эмоционального реагирования на боль у лиц, получивших ожоги, представляется чрезвычайно актуальной как в теоретическом, так и практическом плане.

В целом в отношении личности больных, получающих ожоги, остается много загадок. Хирурги отмечают значительное обновление контингента больных, среди которых большой удельный вес составляют маргинальные личности (социально отверженные, бомжи, хронические алкоголики и проч.). На бытовом уровне их травматизм получает рациональное объяснение: неосторожное обращение с огнем, курение в постели, алкоголизация и др.

Составленный нами портрет пациента с бытовой ожоговой травмой на основании 90 наблюдений указывает на низкий культурно-образовательный уровень, проблемы социальной адаптации, обусловленные личностными особенностями психопатического склада или явно акцентуированного характера, часто возбудимого типа. Пациента отличает завышенная самооценка, легкость в принятии решений, отсутствие разборчивости в контактах, бесцеремонность поведения, снисходительное отношение к своим промахам и недостаткам, легко возникающие эмоциональные всплески с быстрой отходчивостью, непостоянство в привязанностях. Он, как правило, не склонен контролировать события своей жизни, считая, что от него лично мало что зависит (экстернал), в ряде случаев фаталист. Уклоняется от принятия личной ответственности, в том числе и в отношении своего здоровья. Установлена высокая потребность в алкоголизации как средству снятия напряжения, высоко вероятно наличие зависимости от алкоголя. В большинстве случаев получает ожоги не по неосторожности, а «по глупости» — в ситуации бытовой ссоры, алкогольного опьянения, засыпания с непотушенной сигаретой и др. Отношение к болезни характеризуется широким спектром, однако доминирующими типами отношения к болезни у мужчин является эгоцентрический, а у женщин — тревожный и сенситивный типы реакций. Эгоцентрический тип реагирования характерен для пациентов с нарушениями социальной адаптации (одиноких, не имеющих семьи и работы и др.), который на поведенческом уровне проявляет себя рентными установками, «уходом в болезнь», в ряде случаев — аггравацией симптоматики. Для таких пациентов процесс лечения и пребывание в ожоговом стационаре является компенсаторным способом получения внимания и заботы, которых они, по-видимому, лишены в обычной жизни. Для женщин характерны такие личностные особенности, как обидчивость, ранимость, аффективная ригидность. Они также отличаются низкой способностью устанавливать устойчивые партнерские отношения, не способны к глубокой любви и привязанности, предпочитая поверхностные контакты, от чего бессознательно глубоко страдают. Ожоговая травма является для них в ряде случаев своеобразным самонаказанием за тот образ жизни, который они ведут [31].

Подобные выводы делают и некоторые отечественные психотерапевты, работающие с ожоговыми больными [14; 26; 27; 28]. Изучение анамнеза пролеченных ими пациентов показало, что большинство из них до получения травмы имели длительные психотравмирующие ситуации: развод, затяжные семейные конфликты, финансовые расстройства, внутриличностные духовные кризисы. В связи с этим авторы этих публикаций осторожно констатируют, что, возможно, травма являлась для таких больных патологическим выходом из неразрешённой ситуации. Указанная точка зрения близка к психоналитической, высказанной еще Ф. Александером о том, что получаемые людьми травмы далеко не случайны и детерминированы бессознательными мотивами, вынуждающими их вести себя так, чтобы произошел несчастный случай [3, с. 250–259].

В настоящее время существуют различные классификации боли при ожогах. Традиционно выделяют острую и хроническую боль [20; 24; 40] (и др.). Summer G.J. et al. выделяют следующие виды боли: процедурная боль (первичная механическая гиперальгезия) — возникает при хирургической обработке раны, перевязках и прочих манипуляциях, описываемая как «горение и жжение»; фоновая боль, развивающуюся у пациентов с высокой тревожностью (продолжительная жгучая или пульсирующая боль, которая присутствует даже когда пациент бывает относительно неподвижен); «прорывная боль», появляющаяся при движениях (описывается как жалящая, колющая, стреляющая и стучащая) [40].

Цель данного исследования — изучение эмоционального реагирования на процедурную боль, возникающую во время процедур перевязок у мужчин и женщин — пациентов ожогового отделения.

Материалы и методы

Обследовано (при участии студентки БФ ПГНИУ, медицинской сестры ожогового отделения У.А. Новоселовой) 60 больных с бытовой ожоговой травмой (30 мужчин и 30 женщин) в возрасте от 20 до 40 лет, проходивших лечение в ожоговом отделении Городской больницы № 1 им. академика Е.А. Вагнера г. Березники Пермского края. Социальный статус работающего имели 30 чел., из них 5 мужчин и 25 женщин, главным образом работники малобюджетной сферы: дворники, слесари, сантехники, продавцы. Остальные пациенты нигде не работали. Образовательный уровень испытуемых был сравнительно невысоким: среднее образование имели 19 чел, средне-специальное (ПТУ, техникум) — 41 чел. Холостых и незамужних было 7 чел. (11 %) — 3 мужчины и 4 женщины; состоящих в браке — 17 (28%) — 9 мужчин и 8 женщин; разведенных 26 чел. (43%), из них 13 мужчин и 13 женщин; вдовых 10 чел. (16%) — 5 мужчин и 5 женщин. Более половины обследованных не имели постоянного места жительства.

Почти все испытуемые получили термические ожоги в быту, чему способствовало неосторожное обращение с огнем и быстро воспламеняющимися веществами (газ, аэрозоли, нефтепродукты), сопутствующая алкоголизация, курение в постели. В течение первых трех дней после травмы обратились 57% пострадавших женщин и 28% мужчин. Причинами позднего поступления являлись: субъективная недооценка пострадавшим тяжести полученной травмы, снижение болевой чувствительности в зоне глубокого ожога, нежелание госпитализироваться, беспомощность (среди одиноких), алкоголизация, прием наркотиков и проч.

Больные имели различную локализацию, степень, и площадь ожога (табл. 1).

Как видно из табл. 1, наибольшей ожоговой травматизации у обеих групп подвергались открытые части тела, а у женщин более уязвимыми частями тела являются грудь и туловище.

В работе использовались следующие диагностические методы: 1) опрос медицинского персонала (3 врача и 12 медицинских сестер) о реакциях на боль мужчин и женщин с ожогами во время лечения, процедур и выздоровления; 2) наблюдение, имеющее целью изучение проявлений эмоционального реагирования во время процедур (перевязок) в первые 7 дней поступления в стационар (до оперативного вмешательства), осуществляемое врачом-комбустиологом и операционной медсестрой. Единицами наблюдения являлись: а) вербальные реакции: слова, высказывания; б) выбор испытуемыми слов-дескрипторов, описывающих боль из перечня: плач, страх, страдание, злость, гнев, отвращение, утомление, тревога; в) невербальные реакции: жесты, позы, мимика, выражение лица. Все указанные реакции фиксировались в специальном бланке, учитывалась их частота проявления; 3) Вербально-цветовая шкала интенсивности боли из Многомерного вербально-цветового болевого теста (МвцБТ). Измерительную основу шкалы составляет вербально-рейтинговая шкала, а также цветовая шкала, содержащая цвета, аналогичные 8-цветному тесту Люшера [1].

Результаты

Опрос медперсонала показал, что женщины-пациентки выносливее переносят боль во время перевязки. Медицинские сестры ожогового отделения особо отмечали различия во внешних проявлениях реагирования на послеоперационные процедуры между мужчинами и женщинами. Мужчины реагируют агрессивно, несдержанно, злобно, не могут терпеть боли во время перевязок, способны нецензурно выражаться в адрес медперсонала, пытаются обвинить, и даже ударить. Сотрудники подчеркивают у мужчин безответственность, циничность, наглость и неуважительное отношение к медицинским работникам. Женщины, наоборот, более терпеливы и молчаливы, склонны жалеть себя и вызывать сочувствие со стороны окружающих.

В результате наблюдения получены качественные различия в способах эмоционального реагирования на боль у мужчин и женщин — пациентов ожогового отделения. В табл. 2 представлены обобщенные результаты наблюдений медицинского персонала за больными во время лечебных процедур.

В табл. 3 приведен сравнительный анализ частоты вербальных реакций у обследованных больных.

Как видно из табл. 3, частотный статистический анализ обнаружил достоверные различия в выборе и количестве способов вербального реагирования на боль между мужчинами и женщинами на 1% уровне значимости. Сходство вербальных реакций на боль проявлялось в неконтролируемых реакциях — стон, крик, вздохи, междометия («ой!», «ох!», «ай!» и др.). Тем не менее, женщины, в сравнении с мужчинами, в 10 раз чаще молчали и в 10 раз реже употребляли нецензурные выражения. Мужчины чаще переходили на крик во время процедур. Способы поведения также отличались у пациентов обоего пола. Мужчины использовали движения и действия, что затрудняло проведение перевязочных мероприятий. Женщины, же напротив, использовали «сдерживающие» стратегии — мимикой, жестами давали понять медперсоналу о силе переживаемой боли. Распределение частот по указанным в табл. 3 характеристикам позволяет утверждать статистическую достоверность различий в невербально-эмоциональном реагировании на 1% уровне значимости.

Для изучения самооценивания пациентами боли во время процедур перевязок применена Вербально-цветовая шкала интенсивности боли, которая позволяет определить характеристики боли у пациента с помощью цветовой экспресс-диагностики. В результате выбора цветов обнаружены специфические характеристики соотношения выбора цвета и оценки его интенсивности, а также мужчины и женщины выбирали разные цвета по описанию своей боли. Количественные данные представлены в табл. 4.

Как видно из табл. 4, 1/3 женщин оценивали процедурную боль как слабую и средней силы, в то время как аналогичное число мужчин определяли боль как очень сильную и невыносимую. Первоначально мы объясняли этот факт различием в степени ожога: II степень ожога имели 17 женщин, III Б степень — такое же количество мужчин. Для того чтобы достоверно уточнить разницу в реагировании на стимул «боль» у мужчин и женщин, были отобраны пациенты, имеющие III (АБ) степень ожога, что составило 13 женщин и 22 мужчины. Однако, несмотря на одинаковую степень травмы, 90,7% мужчин отметили сильную и невыносимую боль (красный, черный), и только 2 чел. как сильную (фиолетовый), тогда как женщины оценили свою боль в характеристиках средней (23,1 %), сильной и очень сильной (76,9%) интенсивности (синий, фиолетовый, красный цвета). Исходя из дополнительных характеристик цветовых отношений [4], можно интерпретировать цветовые выборы мужчин как проявление агрессии, активного противостояния (красный цвет), подавления потребностей (черный), что вызывает бурное неприятие боли и лечебной процедуры. Примечательно, что только в выборах мужчин присутствует черный цвет как отображение негатива, злости на данную ситуацию. Боль во время перевязок ран женщинами оценивается как подавленная (фиолетовый), помогающая (синий), активно-агрессивная, вызывающая эмоциональную нестабильность (красный). Сама процедура вызывает возбуждение, беспокойство, которое сдерживается пациенткой. Статистический анализ подтвердил различие в выборе интенсивности переживаний эмоций между мужчинами и женщинами, имеющими одинаковую III АБ степень ожога: при dƒ = 3 χ² = 7,8, что соответствует 5% уровню значимости отличий.

Выбор испытуемыми слов-дескрипторов эмоций во время процедуры перевязки показан в табл. 5.

Как видно из табл. 5, в общей выборке большинство мужчин в оценке своей боли во время процедур указали на такие эмоции, как злость и гнев, а женщины — страдание и тревогу. В группе с одинаковой степенью ожога — III АБ выявлена та же закономерность, статистически значимо подтвержденная с помощью проверки по критерию χ². Таким образом, анализ данных о самооценке интенсивности процедурной боли у ожоговых пациентов позволяет отметить значительные половые различия.

Обсуждение

Одним из важных аспектов настоящего исследования является ответ на вопрос, в чем состоит различие в эмоциональном реагировании на процедурную боль у мужчин и женщин, и какие факторы определяют эти типы реакций у ожоговых пациентов. В целом, на основе полученных нами результатов напрашивается общий вывод о том, что женщины более терпеливо и стоически переносят болевые ощущения при перевязках, а их диапазон реагирования у 2/3 находится в границах «слабая и умеренная боль», в то время как мужчины реагируют на боль очень бурно, с проявлениями крика, гнева, злобы, вербальной и физической агрессии, оценивая свою боль как «очень сильную» и «невыносимую». Этот вывод противоречит общепринятым представлениям о том, что женщины более чувствительны к боли, труднее переносят острую и процедурную боль, как это было показано на основе цитированных выше источников [2; 11; 38; 52; 53]. В качестве аргументов, позволяющих отстаивать выдвинутую нами точку зрения, можно привести следующие:

1)  в доступной литературе нет сопоставимых публикаций по проблеме оценки половых различий реагирования на боль у ожоговых больных. На их дефицит указывают специалисты в этой области [19; 20];

2)  болевой синдром при ожоговой болезни нельзя рассматривать изолированно, в отрыве от общего состояния организма, клинических признаков септикотоксемии, переживаемого личностью посттравматического стресса (ПТСР), проявления которого у мужчин характеризуются большей представленностью экстрапунитивных реакций, раздражительности, агрессивных действий, в отличие от женщин, более склонных к проявлениям депрессивности и тревожности, формированию чувства вины [12; 18; 19; 25]. В этом плане мы солидаризуемся одним из ведущих специалистов по психологическим проблемам ожоговой травмы С.Г. Лафи, которая утверждает, что одной из наиболее важных особенностей болевого синдрома у тяжело обожженных является действие одновременно с болевым раздражителем психологического и эмоционального факторов, которые необходимо учитывать при оценке поведения больного и отношения его к проводимому лечению [19, с. 118];

3)  несмотря на то, что гендерные различия в реагировании на боль изучены недостаточно, является признанным, что мужчинам более свойственны такие характеристики как злоба, ярость и гнев, и что от умения управлять своим гневом может зависеть исход лечения, по крайней мере, для этой категории обожженных [37];

4)  способность более терпеливо переносить боль женщинами может объясняться также их большим опытом общения с болью, обусловленным физиологическими причинами (например, при беременности и родах). При этом женщины используют большее число разнообразных способов адаптации к боли. Например, они могут использовать тактику драматизации с целью получения социальной поддержки, умеют себя заранее подготовить к боли, тогда как мужчины начиная об этом думать, уже испытывают ее [11; 17; 25];

5)  характер болевого реагирования определяется в значительной мере преморбидными особенностями личности. Как уже было показано выше, пациент с бытовой ожоговой травмой достаточно часто является носителем психопатических и акцентуированных черт характера, отягощен наркологическим анамнезом, имеет низкий уровень социальной адаптации и культурного развития. Ожоговая травма как проявление тяжелого стресса еще больше заостряет эти черты и приводит к полному срыву имеющихся, но слабо сформированных адаптационных механизмов [32];

6)  вполне очевидно, что взрослые пациенты с бытовой ожоговой травмой представляют собой особый контингент, который будет отличаться по многим социальным и личностно-психологическим параметрам от категорий больных с производственной и военной ожоговой травмой, а также случайных жертв стихийных бедствий и катастроф, сопровождающихся термическими повреждениями. Вполне очевидно, что сравнительное изучение болевых проявлений при ожоговой болезни могло бы быть представлено новым разделом науки о боли — комбустиологической алгологией.

Результаты нашей работы позволяют сделать следующие выводы:

1.  Существуют половые различия в эмоциональном реагировании на процедурную боль у больных с бытовой ожоговой травмой, подтвержденные статистически значимо с помощью наблюдения и самооценочно-психодиагностическим исследованием. Эти различия представляют специфический набор вербальных и невербальных паттернов реагирования.

2.  Женщины в большей степени склонны оценивать процедурную боль как слабую и умеренную, ассоциировать ее с чувством тревоги и мученичества (страдания) и терпеливее ее переносить. Мужчины преимущественно оценивают боль при перевязках как очень сильную или невыносимую, реагируют на нее бурно, с проявлениями крика, гнева, злобы, вербальной и физической агрессии. Они отличаются слабой способностью самоконтроля, сами процедуры вызывают у них негативное отношение, враждебность и отторжение.

3.  Указанные особенности необходимо учитывать во время проведения хирургических манипуляций при обработке ожоговых ран, анестезии, а также психотерапевтической работе с данным контингентом больных.

Abstract. Problem statement. A great number of publications is devoted to the pain phenomenon, however, as revealed by the literary search, the pain in burn patients proved to be poorly studied. There are no data on specific response to procedural pain in patients with household burn injury with a view to sex and gender differences. The objective of this research was to investigate the emotional response to procedural pain that arises during dressing procedures in men and women — patients in a burns department. Materials and methods. 60 in-patients with household burn injury (30 men and 30 women) were examined. Methods of survey, observation, verbal and color scale for pain intensity from Multidimensional verbal and color pain test were used in this work. Statistical data processing was carried out by means of Pirson criterion χ². Results of research. The survey of medical staff showed that female patients endure pain during dressings more tolerant. Men react aggressively, violently, spitefully, are not able to tolerate pain during dressings, can talk obscenely to medical staff, try to blame, and even to hit, show irresponsibility, cynicism, disrespect to the medical professionals. Women, on the contrary, are more patient and silent, inclined to self-pity and to excite sympathy in the personnel. The qualitative differences in the ways of emotional response to the pain in both men and women patients in the burns department are obtained. The frequency statistical analysis found significant differences in the choice and number of ways of verbal response to pain between men and women (χ² = 78.09; p < 0.01) and also non-verbal emotional response (χ² = 99.34; p < 0.01). The similarity of verbal reactions to pain was manifested in uncontrolled reactions — moans, cries, sighs, interjections. Women, in comparison with men, were silent 10 times more often and used obscene abuse 10 times less often, they used the "restraining" strategy — with the help of mimicry, gestures let the medical staff know about the intensity of the pain experienced. Men started shouting more often, showed redundant and inappropriate motion activity that hampered dressing actions. The application of verbal and color scale showed that 1/3 women assess procedural pain as mild or of medium intensity while similar number of men defined pain as very severe and intolerable. Similar results were obtained when comparing pain responses in patients of both sexes with the same burn degree III AB (χ² = 7.8; p < 0.05). Statistically significant differences in sex were obtained in the choice of words-descriptors of emotional responses to procedural pain in the total sample of patients (χ² = 31.98; p < 0.01) and in the group with the same degree of burn — III AB (χ² = 54.30; p < 0.01). Most of the men in their assessment of pain during dressing procedures pointed to such emotions as anger and rage, but women — suffering and anxiety. Thus, the analysis of data on self-assessment of the intensity of procedural pain in burn patients allowed noting considerable sex differences. The main conclusion is that women endure pain more patiently and stoically while changing dressings, this conclusion contradicts the current opinion that women are more sensitive to pain, endure acute and procedural pain harder. Conclusion. Characteristics of pain response in patients with household burn injury should be considered in the context of the general state of the organism, the clinical picture of the disease, post-traumatic stress disorder experienced by the personality, pain experience, premorbid personality traits. The problem of need for comparative clinical and psychological researches of patients with household, industrial and military burn injuries, as well as victims of natural disasters and catastrophes accompanied by thermal damages is raised.

The problem of pain and pain relief is central in clinical medicine. At present considerable material has been accumulated on manifestations of transient, acute and chronic pain in various diseases, pathophysiological mechanisms of pain, medicamental, physiotherapeutic, psychological and different non-drug means of its relief.

Intensity and character of painful sensations are the result of activity of two systems of an organism: pain (nociceptive) and anti-pain, anesthetic (antinociceptive) systems. Their interaction with each other determines what kind of pain a person has [4; 7; 13]. According to pain sensitivity types epicritic pain and protopathic pain are distinguished. Epicritic ("first", "fast", " warning ") pain results from the action of irritants of low-level and middle-level intensity. Protopathic ("ancient", "slow", "burdensome") pain arises under the influence of intense, "destructive", "large-scale" irritants.

The International Association for the Study of Pain (IASP) gives the following definition of pain: "an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage" [44; 50]. This definition indicates that pain is not always caused by tissue damage, as it happens in case of trauma, inflammatory or degenerative processes due to ischemia, but also because of psychological reasons that emphasizes the importance of the personal-subjective factor.

According to the theory of gate control, psychosocial and behavioral factors are integrated into physiological processes of subjective experience (perception, sensation) of pain. It is the subjectivity of feeling pain that means that individual attitude, assessments and coping-strategies make the contribution to maintenance, worsening or weakening of a sensation of pain or pain behavior [48; 49]. The pain component, along with the emotional, intellectual and volitional parties, is a component of the internal picture of the disease (IPD), representing the level of sensations (sensory level) [21; 30]. The subjective sensations are the main source and the indicator of changes in the structure of IPD [29].

The study of pain is a complicated complex of problems. So, patients tell about location, character and duration of pain in different ways. The differences between men and women in response to pain were confirmed by many epidemiological, clinical, and experimental data. According to A. M. Vein, "pain is the sad privilege of women, although sometimes there is an invert correlation" [9]. They are more sensitive to pain and more often than men suffer from pain syndromes [12; 39; 43; 57].

In most cases it is found out that women feel pain more intensively than men [11; 44]. Similar regularities also occur between girls and boys in childhood [46; 47]. It is also found out that men and women differ greatly in relation to pain expectation. According to gender stereotypes (norms), representatives of both sexes believe that men are less sensitive to pain, endure pain better and report on pain less willingly [52].

Most works concerning the problem of pain, including those published in specialized national and international journals, provide data mainly about its chronic form (head, spinal, migraine, abdominal pain, etc.). It has been shown that there are differences in control of pain in men and women: men are more inclined to believe that pain can be controlled and demonstrate a low level of catastrophization (feeling of lack of control over the situation, excess of emotion (worry) about the future) [54; 55]. According to some authors, women experience postoperative [38] and procedural pain [53] harder, others say, there are no significant differences in the perception of acute pain [41; 42].

G. A. Adashinskaya, E. E. Meyzerov, S. N. Enikolopov investigated differences in the perception of pain in 41 men and 41 women with a neurogenic, somatogenic and psychogenic pain syndrome, and found that women in comparison with men more than twice often experience severe pain irrespective of the form of pain syndrome, they are characterized by a pronounced affective experience of pain; men express the emotional attitude more pronounced by the "moderate" and "severe" pain, pain of high intensity ("very severe", "intolerable") they are characterized by restraint in emotional assessment; women when assessing pain choose more sensory and emotional descriptors than men irrespective of the form of pain; patients with psychogenic pain syndrome (men and women) for the characterization of pain and emotional-affective relationship to pain using a larger number of descriptors in comparison with other patients [2].

The interrelation between pain and affective responses to pain is proved. Negative emotions themselves increase somatic sensitivity, can cause pain or be the result of chronic pain [22; 51]. W. E. Haley, J. A. Turner & J. M. Romano showed that depression was considerably associated with pain in women, while in men depression detects the connection with the level of activity, but not pain [45]. These and other works point to the high importance of researches in the field of gender algology, however the detailed analysis of all its aspects is not included into our objectives.

The study of the problem of pain is extremely important for combustiology, since all kinds of burns are accompanied by pain. Pain accompanies burns from the first minute until complete healing of wounds. Acute epicritic pain of a burn patient arises at the time of thermal injury and operations and painful manipulations. During the burn disease the patient experiences a protopathic pain, the severity of which depends on the adequacy of carried out anesthesia and intensive therapy at the time of injury, at the subsequent surgical procedures and painful manipulations. Intensity of pain in patients with burns depends on depth and location of lesion, on phase of wound process. The household burn injury currently holds the leading position in the structure of the burn injury (over 75%) [23].

Thermal injury causes a complex of changes manifested as emotional, mental, psychomotor and other types of disorder. Observations show that the psychological state of the burn patient is formed in the process of surgical interventions and dressings. Domestic literature sufficiently describes psychopathological disorders of patients with burns [6; 10; 16; 33; 34; 35], psychological characteristics of traumatic stress in burn disease, its dynamics and consequences [8; 12; 15; 17; 18; 19; 25; 28; 35; 36] (etc). However, as shown by S.G. Lafi and N. M. Lafi, the phenomenon of pain in burn disease is surprisingly out of sight of other researchers [20]. The literary search carried out by the authors of the article let them find only one report on pain syndrome of burn patients in Russian, presented by the Ukrainian experts [24]. In these positions the declared subject of study of emotional response to pain in patients who got burns, is extremely important both in theoretical and practical terms.

In general, concerning personality of patients getting burns, there are still a lot of riddles. Surgeons note considerable renovation of the patient population, among whom large proportions are made by marginalized persons (socially excluded, homeless, chronic alcoholics, etc.). At the household level, their injuries have a rational explanation: careless handling of fire, smoking in bed, alcoholism, etc.

We made a portrait of a patient with household burn injury based on 90 observations; it shows low cultural and educational level, problems of social adaptation, due to the personal characteristics of psychopathic mentality or clearly accentuated character, often of excitable type. The patient is distinguished by high self-rating, ease in decision-making, lack of demands in contacts, impudence of behavior, condescending attitude toward his or her mistakes and shortcomings, easily occurred emotional outbursts with easily appeasing, the instability of affection. As a rule, the patient is not inclined to control the events of his or her life, believing that little depends personally on him or her (an external), in some cases — a fatalist. He or she evades from taking personal responsibility, including one concerning their health. We state a high need for alcohol as a means of stress relief, there must be alcohol addiction. In most cases he or she gets burns not by negligence, but "stupidly" — in a situation of domestic quarrels, alcoholic intoxication, falling asleep with a lit cigarette, etc. The attitude towards the disease is characterized by a wide range, however, the dominant types of attitude to the disease of men is egocentric, and of women — anxious and sensitive types of reactions. The egocentric type of reaction is characteristic for patients with disorder in social adaptation (single, with no family and work, etc.), which at the behavioral level manifests itself in selfish attitude, "immersion in the disease”, in some cases, in aggravation of symptoms. For these patients process of treatment and stay in a burn hospital is a compensatory way of getting attention and care, which they are apparently deprived of in normal life. For women such personal features as sensitivity, vulnerability, affective rigidity are characteristic. They also have low ability to establish stable partnership, unable to deep love and affection, preferring perfunctory contacts, what they unconsciously suffer from deeply. Burn injury is in some cases a kind of self-punishment for the lifestyle that they lead [31].

Similar conclusions are also drawn by some domestic psychotherapists working with burn patients [14; 26; 27; 28]. Studying of the anamnesis of the patients treated by them showed that most of them had long stressful situations before the injury: divorce, prolonged family conflicts, financial frustration, intrapersonal spiritual crises. In this regard, the authors of these publications ascertain carefully that perhaps the injury was a pathological way out of unresolved situations for such patients. This point of view is close to psychoanalytical one expressed by F. Alexander that people get injuries not accidentally but they are determined by unconscious motives compelling them to behave so that an accident occurs [3, p. 250–259].

Currently there are different classifications of pain from burns. Traditionally acute and chronic pain are distinguished [20; 24; 40] (and others). Summer G. J. et al. distinguish between the following types of pain: procedural pain (primary mechanical hyperalgesia) — occurs at debridement, dressings, and other manipulations, described as "burning and burning"; background pain developing in patients with high anxiety (prolonged burning or throbbing pain, which is present even when the patient is relatively immobile); "breakthrough pain" that appears when moving (described as stinging, stabbing, shooting, darting and knocking) [40].

The purpose of this research was to investigate the emotional responses to procedural pain that occurs during the procedures of dressing in men and women patients of a burns department.

Materials and methods

60 patients with household burn injury (30 men and 30 women) aged 20 to 40 years old undergoing treatment in the burns department of the City hospital № 1, named after the academician E. A. Wagner, in the town of Berezniki Perm Region were examined (with the assistance of the student of Berezniki Branch of Perm State Scientific Research University, a nurse of the burns department, U. A. Novoselova). 30 people, 5 men and 25 women, had the social status of employed, mainly employees of the low-income sector: street cleaners, metalworkers, plumbers, shop assistants. The other patients did not work anywhere. Educational level of examinees was rather low: 19 persons had secondary education, 41 people had specialized secondary (technical training college, technical school) education. 7 people (11%) — 3 men and 4 women — were single; 17 people (28%) — 9 men and 8 women — were married; 26 respondents (43%) — including 13 men and 13 women, were divorced; 10 people (16%) — 5 men and 5 women — were widowers and widows. More than a half of surveyed had no permanent residence.

Almost all examinees got thermal burns at home by careless handling of fire and quickly flammable substances (gas, aerosols, oil products), accompanying alcoholism, smoking in bed. During the first three days after the injury 57% of injured women and 28% of men addressed for help. The reasons for late arrivals in hospital were: subjective underestimation by the injured of the severity of the injury,reduced pain sensitivity in the area of deep burns, unwillingness to be hospitalized, helplessness (among single), alcoholism, drug use, etc.

Patients had different location, extent, and area of burns (table 1).

As we can see from table 1, most of the burn traumas in both groups were exposed parts of the body, and more open to injury parts of the body of women were chest and body.

The following diagnostic methods were used in the work: 1) questioning of medical personnel (3 doctors and 12 nurses) about the reactions to the pain of men and women with burns during treatment, medical procedures, and recovery; 2) observation, aiming at the studying of manifestations of emotional response during procedures (dressings) in the first 7 days of arrival in a hospital (before the surgery), performed by a physician combustiologist and a scrub nurse. The observation units were: a) verbal reactions: words, utterance; b) selecting by the examinees words-descriptors describing pain from the list: crying, fear, suffering, distress, anger, disgust, exhaustion, anxiety; c) nonverbal reactions: gestures, postures, mimicry, facial expression. All specified reactions were fixed in a special form, taking into account the frequency of their manifestation; 3) a verbal and color scale for pain intensity from Multidimensional verbal and color pain test (Mdvcpt). The measuring basis of a scale is made up by a verbal and rating scale, and also a color scale containing colors similar to the 8-color test of Lyusher [1].

Results

The questioning of medical staff showed that female patientsendure pain during dressings more tolerant. Nurses of the burns department especially noted differences in external manifestations of response to postoperative procedures between men and women. Men react aggressively, violently, spitefully, cannot endure pain during dressings, can talk obscenely to medical staff, trying to blame, and even to hit. Medical staff emphasizes irresponsibility, cynicism, impudence and the disrespectful attitude of men towards medical personnel. Women, on the contrary, are more patient and silent, inclined to self-pity and to exciting sympathy in people around.

As a result of observation we obtained qualitative differences in ways of emotional response to pain in men and women — patients of the burns department. The generalized results of the observation over patients during medical procedures by the medical personnel are presented in table 2.

Table 3 shows a comparative analysis of frequency of verbal reactions of the patients examined.

As we can see from table 3, the frequency statistical analysis found significant differences in the choice and number of ways of verbal response to pain between men and women at 1% level of significance. The similarity of verbal reactions to pain was manifested in uncontrolled reactions — moan, shout, sigh, ("oh!", "ah!", "ouch!" etc.). Nevertheless, women, in comparison with men were silent 10 times more often and used obscene abuse 10 times less often. Men began shouting during procedures more often. Ways of behavior also differed in patients of both sexes. Men used movements and actions that made the process of dressing more complicated. Women, on the contrary, used the “restraining" strategies — let the medical staff know about the intensity of experienced pain by facial expressions, gestures. Frequency distribution of the characteristics given in table 3, allows asserting statistical reliability of differences in nonverbal and emotional response at 1% significance level.

The Verbal and color scale of pain intensity was applied to study the self-assessment of pain of the patients during dressing procedures which allows to determine the characteristics of pain of a patient by color express diagnostics. As a result of the choice of colors specific characteristics of correlation of a choice of color and assessment of its intensity were found, and men and women chose different colors according to the description of their pain. Quantitative data are presented in table 4.

As we can see from table 4, 1/3 of women assessed procedural pain as mild or of medium intensity, while a similar number of men identified pain as very severe and intolerable. Primarily we explained this fact by the difference in burn degree: 17 women had burn degree II, the same number of men had degree III B. In order to specify the difference in response to stimulus "pain" of men and women, the patients having burn degree III (AB) were selected those were 13 women and 22 men. However, despite the same degree of injury, 90.7 per cent of men indicated severe and intolerable pain (red, black), and only 2 men — severe pain (violet), whereas women rated their pain in the characteristics of medium intensity (23.1 %), severe and very severe (76.9 %) (blue, violet, red color). Based on additional characteristics of the color relations [4], it is possible to interpret color choice of men as manifestation of aggression, active confrontation (red color), suppression of needs (black) that causes violent aversion to pain and medical procedure. It is worth noting that only men chose black color as an indicator of negative attitude and rage to this situation. Women assessed their pain during wounds dressings as suppressed (violet), helping (blue), active and aggressive, causing emotional instability (red). The procedure itself causes excitement, anxiety, which is restrained by the female patient. The statistical analysis confirmed the difference in the choice of intensity of feeling emotions between men and women with the same burn degree III AB: when dƒ = 3 χ² = 7.8, which corresponds to a 5% significance level of differences.

As we can see from table 5, in the general selection most of the men estimating pain during procedures showed such emotions as rage and anger, and the women — suffering and anxiety. In the group with the same burn degree III AB the same regularity is revealed which is statistically confirmed by means of checking by criterion χ2. Thus, the analysis of data on a self-assessment of procedural pain intensity of burn patients allows noting considerable sex differences.

Discussion

One of important aspects of this research is to answer the question, what is the difference in emotional responses to procedural pain between men and women, and what factors determine these types of reactions of burn patients. In general, on the basis of the results obtained we can come at a general conclusion that women endure dressing pain more patiently and fearlessly, and the range of reaction of 2/3 of them is in limits "mild and moderate pain", while men react to pain very roughly, with demonstration of shout, anger, rage, verbal and physical aggression, assessing the pain as "very severe" and "intolerable". This conclusion contradicts the current opinion that women are more sensitive to pain, endure acute and procedural pain harder as it was shown on the basis of the above-cited sources [2; 11; 38; 52; 53]. As arguments proving our point of view, we can mention the following:

1)  in the available literature there is no comparable published works on the problem of assessment of sex differences of burn patients in response to pain. Their deficiency is pointed out by the experts in this field [19; 20];

2)  the pain syndrome of a burn disease cannot be considered separately, in isolation from the general condition of an organism, from the clinical signs of a septicotoxemia, post-traumatic stress disorder (PTSD) experienced by the personality, symptoms of which in men are more represented by extrapunitive reactions, irritability, aggressive actions, unlike women which are more inclined to symptoms of depression and anxiety, formation of sense of guilt [12; 18; 19; 25]. In this regard, we associate ourselves with one of the leading experts on psychological problems of burn injury S. G. Lafi who asserts that one of the most important features of pain syndrome in hard burned patients is action simultaneously with painful irritant of psychological and emotional factors which must be considered when assessing the patient's behavior and his or her attitude to therapy [19, page 118];

3)  in spite of the fact that gender differences in response to pain are studied insufficiently, it is recognized that such characteristics as spite, rage and anger are more peculiar to men, and that the treatment result, at least for this category of burned, may depend on ability to control their anger [37];

4)  the ability of women to endure pain more patiently can be also explained due to their wide experience of intercourse with pain, caused by physiological reasons (for example, pregnancy and childbirth). Thus women use a greater number of different ways of adaptation to pain. For example, they can use the tactics of dramatization for the purpose of receiving social support, they are able to prepare themselves for pain in advance, whereas men are experiencing pain while starting to think about it [11; 17; 25];

5)  the nature of the pain response is determined largely by premorbid personality characteristics. As shown above, the patient with household burn injury rather often is the carrier of psychopathic and accentuated traits of character, is burdened with a drug history, has a low level of social adaptation and cultural development. The burn injury as manifestation of a heavy stress sharpens these traits even more and leads to a complete breakdown of the existing, but poorly formed adaptive mechanisms [32];

6)  it is quite obvious that adult patients with household burn injury represent a special contingent, which differ in many social and personal and psychological parameters from categories of patients with industrial and military burn injury, and also from the accidental victims of natural disasters and catastrophes which are accompanied by thermal damages. It is quite obvious that comparative study of manifestations of pain in burn patients could be represented by a new section of the study of pain — combustiological algology.

 

The results of our work let us draw the following conclusions:

1.   There are sex differences in emotional responses to procedural pain in patients with household burn injury, confirmed statistically significant by means of observation and self-assessing psychodiagnostic testing. These differences present a specific set of verbal and nonverbal patterns of reaction.

2.   Women are more inclined to assess procedural pain as mild and of medium intensity, to associate it with feelings of anxiety and torment (suffering) and to endure it more patiently. Men mainly assess the pain when dressing as very acute and intolerable; react roughly, with manifestations of shout, anger, rage, verbal and physical aggression. They have poor ability of self-control, the procedures themselves cause negative attitude, hostility and rejection.

3.   The specified features should be considered during surgical manipulations by the treatment of burn wounds, by anesthesia, and also psychotherapeutic work with this contingent of patients.
