Специфика когнитивного дефицита у детей и подростков при шизофрении: клинические и возрастные факторы

Аннотация. Введение. Исследования когнитивных дефицитов при шизофрении у взрослых широко известны, однако немного исследований посвящено изучению этого вопроса у детей и подростков. Цель: оценка видов и степени тяжести когнитивных дефицитов у детей и подростков при шизофрении в связи с клиническими факторами (диагноз, возраст начала заболевания), а также возрастом и полом больных. Материал и методы: патопсихологическое обследование 207 больных (49 пациентов с детской шизофренией, 119 пациентов — с шизотипическими расстройствами, 39 пациентов — с другими формами шизофрении) в возрасте от 7 до 16 лет. Использованы z-шкалы для оценки выраженности дефицитов в памяти, внимании, мышлении. Выделены 5 типов когнитивного развития — опережающее, нормальное, искаженное, дефицитарное, дефектное. Результаты: значительно выраженные нарушения внимания получены по всем группам пациентов. Преобладающее большинство больных имели дефицитарный вариант когнитивного развития по всем трем когнитивным процессам. Кластеризация третьего уровня позволила оценить отношения между когнитивными дефицитами, степенью их выраженности, полом, возрастом и клиническими факторами (диагноз, возраст начала). Между мальчиками и девочками не было получено значимых различий по когнитивным дефицитам, но получены — по возрасту начала — мальчики заболевают раньше. Выводы: полученные данные подтверждают значимость клинических факторов (диагноз, возраст начала) в проблеме когнитивных дефицитов, особенно это важно для других показателей — пола и возраста больных детей и подростков с шизофренией.

Введение

Рост интереса к проблеме когнитивного дефицита при шизофрении в последние годы во многом обусловлен стремлением исследователей найти точные средства и способы оценки его проявлений. Такой подход может помочь выявить адекватные терапевтические мишени в лечении этого заболевания, специфические эндофенотипы [21].

Исследователи выделяют множество факторов, вносящих свой вклад в появление признаков когнитивного снижения у больного. Среди них стоит отметить связь с возрастом начала заболевания. Некоторую сложность в оценке подобной связи вносит трудность разделения начала проявления неспецифической и размытой позитивной симптоматики (к которой нередко относят и когнитивные расстройства) в продромальном периоде с началом полноценного синдрома [28]. Однако многие десятилетия исследований подтверждают, что такая связь существует и проявляется в более тяжелом исходе при более раннем начале [3; 23]. Часто исследователи указывают на связь с полом — более раннее начало шизофрении отмечается у мужчин [17; 18].

Другим фактором, прогнозирующим тяжесть заболевания, является связь с симптоматикой в преморбиде. Отмечается, что чем более выраженной является симптоматика до начала заболевания, тем тяжелее и в более раннем возрасте ожидается начало психоза [19]. Шизофрения, начавшаяся в детском или подростковом возрасте, часто рассматривается как более тяжелая форма заболевания. При этом часто указывают на некоторую генетическую предиспозицию [21], в значительной мере определяющую течение и исход заболевания таких больных. Среди признаков тяжести заболевания обычно указывают на серьезные отклонения в развитии нервной системы в преморбиде, более плохой исход в долгосрочной перспективе, цитогенетические аномалии и потенциально бóльшую вероятность наличия шизофрении и в семейной истории по сравнению со взрослыми больными [24].

Развитие описанного выше направления в зарубежных исследованиях поставило задачу разработки средств оценки состояния когнитивной сферы больных и привело к появлению таких диагностических средств, как, например, MATRICS Национального института психического здоровья США [26; 27]. В отечественной патопсихологии оценка когнитивной сферы больных традиционно опирается на деятельностный подход, и поэтому ее методы отличаются от зарубежных. Такая оценка направлена на качественную квалификацию состояния когнитивной сферы, подчеркивающую ее своеобразие у конкретного больного, в этих случаях основными являются задачи определения степени выраженности когнитивного снижения по отношению к здоровым сверстникам.

В работах отечественных психологов было показано наличие особого варианта познавательного развития детей при шизофрении [7; 8]. Сохранные и измененные параметры когнитивных процессов (мышления и восприятия) у больных шизофренией детей и подростков отражают имеющуюся диссоциацию психического развития. На следующем этапе работ, проводимых отделом медицинской психологии НЦПЗ РАМН, гармоничность когнитивного развития изучалась на примере соотнесения операционных компонентов мыслительной и перцептивной (зрительное восприятие) деятельности, применялась кластеризация данных по 86 больным детям и подросткам с расстройствами круга шизофрении. Получены данные кластеризации 2 и 3 иерархического уровня. Содержательно кластеризацию 2-го уровня можно интерпретировать как разные варианты рассогласованного уровня развития мышления и восприятия у данных испытуемых. По-видимому, имеется вариативное проявление диссоциации операционного компонента мышления и восприятия. Подобный вывод делался в предыдущих исследованиях отдела медицинской психологии НЦПЗ РАМН на основании сравнения средних показателей, без выделения различных видов рассогласования уровня развития операционного компонента мышления и восприятия [4; 8]. На третьем уровне иерархии структура кластеров была интерпретирована как два основных проявления когнитивного дизонтогенеза — преимущественно искаженного (кластер 2) и преимущественно задержанного типов (кластер 1), в соответствии с описанными клиницистами типами дизонтогенеза при шизофрении в детском возрасте — задержанный и искаженный типы по О.П. Юрьевой [15]. Кластеры третьего уровня иерархии могут различать параметры, прямо не связанные с оценкой состояния операционного компонента мышления и зрительного восприятия. К таким параметрам можно отнести длительность заболевания, время его начала, а также степень прогредиентности. Квалификация когнитивного дизонтогенеза при шизофрении в детском возрасте с применением аппарата кластерного анализа открывает новые возможности в количественной и качественной оценке разных параметров психического дизонтогенеза [5; 12]. В исследованиях отдела также были выделены и описаны разные варианты выраженных когнитивных дефицитов (например, искаженный, дефицитарный и регрессивно-дефектирующий виды когнитивного дизонтогенеза в работе А.А. Коваль-Зайцева [6]), динамики познавательного развития при расстройствах шизофренического спектра [14].

Как видно из обзора, при всей разработанности темы когнитивных расстройств при шизофрении в центре внимания остаются преимущественно взрослые больные, имеющие стаж болезни, и обусловленный ею определённый уровень когнитивного снижения [1; 10; 13; 20; 22]. Гораздо меньшее внимание в исследованиях по данной проблематике посвящено оценке когнитивного дефицита больных детского и подросткового возраста, когда болезненный процесс накладывается на формирующуюся когнитивную сферу. Настоящее исследование интегрирует в себе научные достижения отдела медицинской психологии НЦПЗ РАМН с учетом тенденций зарубежных исследований и посвящено изучению клинических и возрастных параметров когнитивного дизонтогенеза.

Цель работы: исследование специфики когнитивного дефицита у больных детей и подростков.

Задачи исследования:

•   разработка способов оценки проявлений когнитивного дефицита;

•   описание типов и степеней выраженности когнитивного дефицита;

•   оценка влияния факторов диагноза и возраста начала заболевания на проявления когнитивного дефицита.

Характеристика материала исследования

В исследование вошли данные патопсихологического обследования детей и подростков, проходивших стационарное лечение на базе 7-го клинического отделения ФГБУ «НЦПЗ» РАМН в период с марта 1996 по декабрь 2011 года. Патопсихологическое обследование проводилось близко к концу госпитализации в период стабильного состояния больного. Терапия подбиралась лечащим врачом индивидуально в соответствии с состоянием пациента. Выборка составила 207 больных, половозрастные характеристики которых представлены в таблице 1. Диагнозы в соответствии с рубриками по МКБ-10, поставленные лечащими врачами, были взяты нами из истории болезни и распределены по трем группам:

1)   группа больных с диагнозом шизофрения, детский тип (F20.8): больные с шизофренией, начавшейся в детском возрасте (рубрика F20.8x по МКБ-10), а также с диагнозами детский аутизм, атипичный аутизм (рубрика F84);

2)   группа больных с диагнозом шизотипическое расстройство (F21): больные с диагнозами психопатоподобное, неврозоподобное, личностное шизотипическое расстройство;

3)   группа больных с диагнозами шизофренического спектра (F2x.x): больные с уточненными (гебефренная, кататоническая, параноидная) и неутонченными формами шизофрении, а также больные с диагнозами из других рубрик шизофренического спектра (шизоаффективное расстройство). Среди больных, имевших диагнозы, выставленные по МКБ-9, в эту группу вошли больные с диагнозами острый шизофренический эпизод (295.4).

Анализ историй болезни позволил нам получить еще один показатель — давность заболевания. На основе этих данных мы смогли получить значения возраста начала заболевания как разность возраста на момент обследования и давности заболевания в годах. Однако эти сведения остались неуточненными для 12 человек из нашей выборки (табл. 2).

Из табл. 2 видно, что в среднем мальчики начинают болеть раньше девочек. Достоверные различия наблюдаются как между мальчиками и девочками из группы F20.8, так и во всей выборке больных в целом.

Для оценки когнитивного дефицита нами использовались данные патопсихологического обследования здоровых сверстников 7—16 лет, собранные сотрудниками лаборатории медицинской психологии ФГБУ «НЦПЗ» РАМН и их студентами в период с июля 1997 по апрель 2013 в средних учебных заведениях г. Москвы. На основании этих данных нами были получены нормативные возрастные показатели развития познавательных процессов, необходимые для определения уровня когнитивного дефицита больных.

Методы

Патопсихологическая диагностика. Диагностические данные по трем познавательным процессам (ПП): память, внимание и мышление были получены с помощью соответствующей патопсихологической методики, в каждой из которых оценивался один параметр:

•   память — объем непосредственной слухоречевой памяти (10 слов);

•   внимание — время выполнения первой таблицы Шульте;

•   мышление — коэффициент стандартности (методика Конструирование объектов).

Количественная оценка когнитивного дефицита у больных проводилась с применением Z-шкал. Значения Z-шкал были получены на основе имеющихся нормативных показателей как разность показателя больного с нормативным средним, отнесенным к нормативному стандартному отклонению. Z-значения вычислялись индивидуально для каждого больного с учетом его возраста с точностью до 1 года: чем меньше полученное Z-значение, тем больше дефицит. Тем самым были получены три показателя когнитивного дефицита:

•   ZНП — дефицит объема слухоречевой памяти;

•   ZТШ — дефицит времени выполнения первой таблицы Шульте;

•   ZКС — дефицит по коэффициенту стандартности мышления.

Бездефицитарным («нормой») было решено считать диапазон −1 ≤ Z ≤ 1 (т.е. ±1 стандартное отклонение от нормативного среднего); показатели ниже этого диапазона считались дефицитом, выше — опережением в познавательном развитии.

Для каждого больного были получены сочетания показателей дефицита по оцениваемым ПП с различной степенью его проявления (величина Z). Это позволило говорить о различных степенях выраженности дефицита и вариантах когнитивного развития (в том числе дефицитарного) обследованных больных.

Степень выраженности когнитивного дефицита каждого больного оценивалась следующим образом (рис. 1):

•   выраженный дефицит — отставание от нормы на два и более стандартных отклонения (Z < −2);

•   умеренный дефицит — отставание от нормы в пределах от одного до двух стандартных отклонений (−2 < Z < −1);

•   бездефицитарное состояние ПП (норма развития) — не наблюдается отклонения от нормы (Z > −1).

Варианты когнитивного развития оценивались способом «ранжирования» континуума познавательного развития «норма — дефициты — дефект» на основе соотношения  показателей  ZНП,  ZТШ  и  ZКС с возрастной нормой: в норме (−1,0 ≤ Z ≤1,0), отстают (Z < −1,0) или опережают (Z >1,0) ее. Были получены 5 вариантов когнитивного развития (в трех случаях это развитие представляет собой варианты дефицита): опережающий, нормальный, искаженный, дефицитарный и дефект (табл. 3). В соответствии с нашими представлениями о когнитивном дизонтогенезе, к нему из выделенных вариантов познавательного развития могут быть отнесены 3, 4 и 5 варианты (см. табл. 3).

Кластерный анализ. С целью определения специфики дефицита отдельных познавательных процессов и их вклада в общую картину когнитивного дефицита больных, нами был проведен кластерный иерархический анализ методом средних связей. Анализ был основан на показателях ZНП, ZТШ и ZКС и проводился отдельно для каждой диагностической группы. В силу наличия экстремальных значений по показателю ZТШ (−32,1; −28,7; −15,9)один больной из группы F21 и двое больных из группы F20.8 были исключены из кластерного анализа. В результате была выбрана модель, включающая 3 кластера для каждого диагноза. Далее кластеры сравнивались между собой по следующим параметрам: представленность в кластере выраженной степени дефицита, пол (с помощью статистики χ²), возраст и возраст начала заболевания больных (с помощью однофакторного дисперсионного анализа и попарного сравнения Бонферрони) для каждого диагноза. Выбор только выраженной степени дефицита для описания кластеров был обусловлен тем, что умеренная степень дефицита может частично объясняться индивидуальным разбросом уровня выполнения методик испытуемыми, тогда как выраженный дефицит с большей вероятностью может быть обусловлен болезнью.

Результаты исследования

Рассмотрим общую картину дефицитов больных. На рис. 2 представлены средние значения по трем показателям дефицита для каждого диагноза. Дефицит внимания (красный цвет на рис. 2) заметно преобладает во всех диагностических группах по сравнению с дефицитами памяти и мышления и не различается в среднем между диагнозами (F(2,204)=0,060; p=0,942), что, по нашему предположению, может быть обусловлено наличием лечения. Дефицит памяти (синий цвет на рис. 2) достоверно различается между всеми диагнозами (F(2,204)=6,711; p=0,002), более всего выражен в группе F20.8 (−1,36), а меньше всего — у F2x.x (−0,61). Дефицит стандартности мышления (зеленый цвет на рис. 2) наиболее выражен в группе F20.8 (−1,36), что достоверно ниже, чем у F21 (p=0,014).

Тем самым, в среднем дефицит внимания для F20.8 и F21 достигает выраженной степени, для F2x.x — умеренной. Дефициты же памяти и внимания в среднем укладываются в норму для F21 и F2x.x, тогда как для F20.8 являются умеренными по степени выраженности.

Варианты когнитивного развития (дефицита). Оценка вариантов когнитивного развития больных, представленная в табл. 4, показала, что для каждого диагноза характерна специфическая картина распределения по типам дефицита.

Наиболее представленным для всех больных является дефицитарный тип когнитивного развития, для группы F20.8 — это 63% больных, для группы F21 — 65%, для группы F2x.x — 77%. Наименее представлен опережающий тип, который оказался не характерен для группы F2x.x. Различия в частоте распределения диагнозов в каждом типе оказались достоверными для нормы (p<0,0011), дефицитарного типа (p=0,007) и для дефекта (p<0,001). Дефект представлен у 24% больных из группы F20.8, что значимо больше, чем в группе F21 (p<0,001) и F2x.x (p<0,001). Больше всего нормативно развивающихся пациентов в группе F21 — 14% (значимые различия с группой F20.8 (p<0,001) и F2x.x (p<0,001)). Искаженный тип чаще встречается в группе F2x.x по сравнению с F20.8 (p=0,083) и F21 (p=0,048). В группе F2x.x дефицитарный тип также встречается чаще, чем в двух других (значимые различия с группой F20.8 (p=0,026) и F21 (p=0,003)).

Иными словами, специфика картины когнитивных нарушений во многом определяется диагнозом: в группе F20.8 наблюдается «смещение» больных к полюсу дефекта; в группе F21 большинство больных тяготеет к дефицитарному типу и меньшая часть к норме; группа F2x.x занимает промежуточное положение за счет наиболее представленных дефицитарного и искаженного типов.

Степень выраженности дефицита оценивалась для восьми возможных вариантов сочетаний трех познавательных процессов: дефициты отдельных ПП — 1) память, 2) внимание, 3) мышление; сочетания дефицитов по разным ПП — 4) память + внимание, 5) память + мышление, 6) внимание + мышление, 7) память + внимание + мышление; 8) дефицит отсутствует по любому из познавательных процессов. Рассмотрим подробнее полученные данные только для выраженной степени дефицита. На рис. 3 описанные выше сочетания представлены для выраженных дефицитов во всех диагностических группах. Видно, что для большинства больных в каждом диагнозе характерно наличие либо отдельных дефицитов выраженной степени, либо их сочетаний: суммарно это 71% больных в группе F20.8, 59% — в группе F21 и 56% — в группе F2x.x. Наиболее представленным среди выраженных оказывается дефицит внимания для всех диагнозов. Варианты сочетаний выраженных дефицитов в группе больных F20.8 вместе составляют 28,6%, тогда как у двух остальных укладываются в 15% (12,6% у F21 и 10,3% у F2x.x).

Среди отдельных дефицитов умеренной степени на первом месте мышление, затем — дефицит памяти, также во всех группах больных. По суммарному числу сочетаний дефицитов больные из группы F20.8 (20,4%) также преобладают над двумя другими группами больных (12,6% у F21, 10,3% у F2x.x). Бездефицитарное развитие по всем трем анализируемым процессам чаще встречается у больных с шизотипическим расстройством (F21) (15%), тогда как у обеих групп с разными вариантами шизофрении он составляет лишь 4—5% (различия значимы на уровне p≤0,01 по критерию Фишера). При этом среди больных F20.8 более всего пациентов имеют ту или иную степень когнитивного дефицита (29%).

Рассмотрим данные кластерного анализа (КА). Три выделенных кластера в каждой диагностической группе позволили представить более детальную картину специфики когнитивного дефицита и возможную связь с рядом факторов, таких как пол, возраст и возраст начала заболевания. В табл. 5 представлены средние значения переменных, вошедших в КА, и на основе которых были сформированы кластеры.

В табл. 6 представлены значения переменных, не вошедших в КА, а также общие характеристики кластеров. Из табл. 6 следует, что больные неравномерно распределились по кластерам. Самыми большими по численности оказались кластер 1 для F20.8, кластер 3 для F21 и кластер 2 для F2x.x, в которые попали больные с наиболее похожими друг на друга уровнями дефицитов.

Как видно из табл. 5 и 6 номера кластеров не сопоставимы друг с другом для разных диагнозов и носят условных характер. Рассмотрим кластеры отдельно для каждого диагноза и факторов, не вошедших в КА.

Пол и возраст. Не было получено достоверных различий в распределении мальчиков и девочек по кластерам ни в одной из диагностических групп. Достоверные отличия кластеров по возрасту отмечаются только в группе F21 (F(2, 115)=10,009; p<0,001). Попарное сравнение также показывает, что возраст испытуемых в группе F21 во всех трех кластерах различается достоверно (p<0,001), в среднем разница составляет примерно 1 год (11,1; 12,3 и 13,1 года для 2, 1 и 3 кластеров, соответственно).

Возраст начала заболевания. Достоверные различия были получены только в группе F20.8 между всеми кластерами на уровне тенденции (F(2, 42)=2,996; p=0,061). Попарное сравнение показывает различие 1 (5,9 лет) и 2 (2,5 года) кластеров (p=0,056). Больные F20.8, попавшие в кластер 2, за исключением одного человека заболели в период раннего детства (от 0 до 4 лет) и имеют выраженные дефициты сразу по нескольким познавательным процессам.

Обсуждение

Оценка специфики когнитивного дефицита детей и подростков с психической патологией проводится по отдельным параметрам когнитивного функционирования в сопоставлении со здоровыми сверстниками или со взрослыми больными. Указания на какие-либо специфические особенности когнитивного профиля больных с расстройствами шизофренического спектра, кроме нарушения формального мышления, динамики IQ, различных нейропсихологических особенностей, встречаются редко [2; 9; 11; 16; 25]. Решение вопроса о роли факторов, способствующих формированию дефицита и его особенностей, позволит подойти к механизмам сложного взаимодействия когнитивного развития и болезненного процесса, а именно — когнитивного дизонтогенеза.

Используя даже небольшое количество параметров, оценивающих проявление когнитивного дефицита, возможно сделать определенные выводы об особенностях формирования когнитивной сферы на фоне болезни круга шизофрении.

Полученные результаты указывают на важную роль клинических факторов в этом процессе. Как у взрослых больных, так и у детей и подростков возраст начала заболевания имеет связь с полом испытуемых (см. табл. 2). Как показало наше исследование, мальчики в среднем заболевают на один-два года раньше девочек, причем более выражены эти различия в случае детской шизофрении (почти 3 года). Однако на этом этапе исследования нам не удалось найти четкой связи пола и профиля когнитивного дефицита. По-видимому, можно говорить о разном темпе когнитивного развития/дизонтогенеза у мальчиков и девочек, но о близких вариантах исхода. Нам также удалось подтвердить предположение о связи большей тяжести дефицита с более ранним началом шизофрении. Наиболее явно такая связь прослеживается только у 27% больных детским типом шизофрении нашей выборки, попавших в кластер 2. Однако только в группе с этим диагнозом проведенный регрессионный анализ возраста начала заболевания в качестве зависимой переменной по показателям дефицита позволил получить значимые модели регрессии при включении всех трех показателей дефицита (R²=0,173; p=0,081), дефицита мышления и внимания (R²=0,173; p=0,033) и только дефицита мышления (R²=0,172; p=0,009). Это свидетельствует в пользу прямой связи раннего начала болезни и появления когнитивного снижения, в первую очередь, мышления, в случае шизофрении.

Другим важным клиническим фактором, определяющим специфику когнитивного дефицита, является нозология. Обнаружены отчетливые различия в проявлениях когнитивного дефицита между тремя диагностическими группами обследованных больных.

Наиболее тяжелым оказывается состояние познавательной сферы у больных детским типом шизофрении: в их когнитивном развитии чаще наблюдается когнитивный дизонтогенез по типу дефекта, который выражается в сочетании дефицитов различных познавательных процессов. Кроме того, в этой группе доля больных с нормальным уровнем познавательного развития невелика.

Самой благополучной оказывается группа больных шизотипическим расстройством, в которой высока доля больных с нормальным познавательным развитием, а основной вклад в дефицитарность когнитивной сферы вносят проблемы с вниманием. Например, у больного с наибольшим возрастным дефицитом внимания во всей выборке (ZТШ = −32,1), имевшего диагноз F21, были близкие к возрасту показатели памяти и мышления. Такое соотношение дефицитов заставляет задуматься над вопросом о средовом влиянии (в первую очередь, лечении) на состояние тех познавательных процессов, которые являются наиболее подвижными и, следовательно, реактивными. По-видимому, особенности внимания в большей степени определяются психическим состоянием испытуемого, чем гораздо более устойчивый познавательный процесс — мышление. Нарушения мышления, в свою очередь, в большей степени формируются под влиянием длительного заболевания.

Наконец, больные прочими формами шизофрении занимают промежуточное положение между двумя другими диагнозами, имея самый высокий процент искаженного типа когнитивного развития, что снижает долю нормально развивающихся больных в данной группе, и выражается в умеренном дефиците мышления. Особый статус этой группе придает более взрослый возраст больных (в среднем они на 1,25 года старше остальных больных) и более позднее начало заболевания (в среднем на 3,8 года позже других больных). Можно предположить, что начало болезни приходится у них на тот момент, когда уже имелся некоторый период нормального, не затронутого болезнью познавательного развития, и текущий болезненный процесс способствует искажению уже частично сформированных когнитивных процессов.

Выводы

1.   Представлен способ оценки когнитивного дефицита у детей и подростков с расстройствами шизофренического спектра с использованием Z-шкал, выделены виды когнитивного развития (от опережающего до грубо дефицитарного), встречающиеся при этой патологии, установлена их частота.

2.   Для всех групп больных наиболее часто встречающимся является дефицитарный тип когнитивного развития с преимущественным снижением показателей познавательных процессов по сравнению с нормой развития (67%), наиболее редким — опережающее когнитивное развитие (2%).

3.   Показатели внимания оказываются наиболее чувствительными для оценки имеющихся когнитивных дефицитов вследствие большей степени отражения состояния больного.

4.   Наиболее тяжелые формы когнитивного дефицита встречаются при детском типе шизофрении, они имеют связь с более ранним возрастом начала заболевания.

Abstract. Background: Researches of cognitive deficits in schizophrenia are mostly conducted on adult patients. A limited number of studies deal with children and adolescents. Purpose: To evaluate types and degrees of cognitive deficit in children and adolescents with schizophrenia in comparison with healthy peers; to estimate the role of clinical (diagnosis, age of onset), gender and age factors. Methods: We used the pathopsychological data of 207 patients (49 with child form of schizophrenia, 119 with schizotypal disorder and 39 with other forms of schizophrenia) at the age from 7 to 16 to assess memory, attention and thinking deficits in z-scales. Five types (outstripping, normal, distorted, deficient and defect) and three degrees of severity (significant, moderate and normative) of deficit were obtained. Particularity of cognitive deficits was examined with cluster analysis. Results: Considerable impairment in attention was obtained in all patients' groups. The majority of patients had the deficient type of cognitive deficit and a significant degree of deficits in all three cognitive domains. Three clusters model revealed the relationship between combination of cognitive deficits with different severity degree and clinical factors (diagnosis, age of onset) and age. No differences were found between boys and girls in cognitive deficits. Conclusion: These findings prove the importance of clinical factors (diagnosis and age of onset) in cognitive deficit particularity relative to age and gender in children and adolescents with schizophrenia.

Introduction

The rise of interest to the problem of cognitive deficit in schizophrenia in recent years comes from researchers’ efforts to find the exact means and methods of evaluation of its manifestations. Such approach can help to identify adequate therapeutic targets for this disease, specific endophenotypes [21].

Researchers have identified many factors that contribute to the development of signs of cognitive decline in the patient. Of them, the connection with the age of the onset of the illness should be particularly noted. Some handicaps in assessing such a connection comes from the difficulty of separating the nonspecific and diffuse positive symptoms (which often carry and cognitive disorders) onset in the prodromal period from the beginning of the full syndrome [28]. However, decades of research suggest that such a link exists and manifests itself in a more serious outcome with earlier onset period [3; 24]. Often, researchers note the gender factor:  among men the onset of schizophrenia is seen earlier [16; 26].

Another factor predicting the severity of the disease is the premorbid symptoms. It is noted that the more frank symptoms are before the onset, the more severe and earlier is the expected onset of psychosis [19]. Schizophrenia which began in childhood or adolescence is often considered to be a more severe form of the disease. It is often suggested that some genetic predisposition [21] largely determines the course and outcome of these patients. Among the criteria of disease severity, serious distortions in the nervous system development in the premorbid period, worse outcome in the long term, cytogenetic abnormalities and a greater possibility of having schizophrenia in family history compared with older patients are usually indicated [25].

The development of the area in foreign studies described above has put the task of elaboration of tools assessing cognitive state of patients and has led to the emergence of such diagnostic methods such as MATRICS developed in National Institute of Mental Health, USA [23; 27]. In Russian tradition of pathopsychology the cognitive assessment of patients traditionally relies on Activity theory, and therefore its methods differ from the foreign ones. Such an assessment is aimed at the qualitative estimation of cognitive state, emphasizing its uniqueness in patient. In these cases the main problem is the evaluation of degree of cognitive impairment in comparison to healthy peers.

Investigations of Russian psychologists have shown the presence of a special variety of the cognitive development of children with schizophrenia [7; 8]. The existence of both unharmed and distorted parameters of cognitive processes (thinking and perception) in children and adolescents with schizophrenia reflects the existing dissociation of mental development. At the next stage of the work carried out by the Department of Clinical Psychology of the MHRC of RAMS, the harmony of cognitive development has been investigated in interrelation of operating components of mental and perceptual (visual perception) activity; data clustering has been used for 86 children and adolescents with schizophrenia spectrum disorders. The data clustering obtained at the 2nd and 3rd hierarchical level. Clustering at level 2 can be interpreted as different versions of mismatching of developmental levels of thinking and perception in these subjects. Likely, there is a variance in dissociation of the operational component of thinking and perception. Similar conclusions were made from previous studies of the department of clinical psychology of MHRC RAMS by comparing the averages, without isolating the different kinds of mismatch of the level of development of the operational component of thinking and perception [4; 8]. The third level of the hierarchical structure of the clusters has been interpreted as the two major manifestations of cognitive dysontogenesis — predominantly distorted (cluster 2) and predominantly detainee types (cluster 1), which is associated with the types of dysontogenesis in schizophrenia in childhood described by clinical specialists — delayed and distorted types by O.P. Yurieva [15]. Clusters of the third level of the hierarchy can distinguish parameters that are not directly related to the assessment of the operational state of the component of thinking and visual perception. These parameters include the duration of the disease, the onset time and the degree of progression. The qualifying of cognitive dysontogenesis in schizophrenia in childhood using the cluster analysis opens up new possibilities in the quantitative and qualitative evaluation of the various parameters of mental dysontogenesis [5; 12]. Investigations of the department also identified and described different variants of cognitive deficits (eg, distorted, deficiency and regressive-defecting types of cognitive dysontogenesis in work of A.A. Koval-Zaitsev [6]), the variants of dynamic of cognitive development in schizophrenia spectrum disorders [14].

As seen from the review, though the topic of cognitive disorders in schizophrenia is widely explored, the investigations have mainly focused on adult patients with experience of illness which causes a certain level of cognitive decline [1; 10; 13; 20; 22]. Much less attention in the research on this issue is devoted to the assessment of cognitive deficits in patients in childhood and adolescence, when the disease process is applied to emerging cognition. The present study integrates the scientific achievements of the department of clinical psychology of MHRC RAMS considering the trends of foreign studies and is devoted to the study of clinical and age-specific parameters of cognitive dysontogenesis.

The aim of the study was to determine the specificity of cognitive deficits in children and adolescents patients.

Research objectives:

•   To develop ways to assess the manifestation of cognitive deficits;

•   Description of the types and degrees of cognitive deficits;

•   Assessment of the impact diagnosis and age of onset factors on the manifestation of cognitive deficits.

Subjects

The study included data of pathopsychological examination of children and adolescents held inpatient treatment based on the 7th of the clinical department FGBI "MHRC" RAMS in the period from March 1996 to December 2011. Pathopsychological survey was conducted near the end of the period of hospitalization in a stable condition of the patients. Therapy was chosen by the attending physician according to the individual patient's condition. The sample consisted of 207 patients. Age and gender characteristics are presented in Table 1. Diagnoses were set by clinicians according to the headings of the ICD-10. It were taken from case histories and divided into three groups:

1)   a group of patients diagnosed with schizophrenia, children type (F20.8): patients with schizophrenia, which began in childhood (heading F20.8x of ICD-10), as well as children diagnosed with autism, atypical autism (category F84);

2)   a group of patients diagnosed with schizotypal disorder (F21): patients diagnosed as psychopathic disorder, neurosis or schizotypal personality disorder;

3)   a group of patients with diagnoses of schizophrenia spectrum (F2x.x): patients with refined (hebephrenic, catatonic, paranoid) and non-refined forms of schizophrenia, and patients with diagnoses of schizophrenia spectrum of other groups (schizoaffective disorder). Among patients who had diagnoses of the ICD-9, this group included patients with acute schizophrenic episode (295.4).

Analysis of case histories allowed us to get another parameter — the disease duration. On the basis of these data we were able to get the value of age of onset as the difference of age at the time of examination and duration of the disease in years. However, these details remain unspecified for 12 people in our sample (Table 2).

Table 2 shows that on average boys start to fall ill earlier than girls. Significant differences in age of onset were observed in group F20.8, and in the entire sample of patients as a whole.

To assess the cognitive deficits we used the data of pathopsychological investigation of healthy children 7—16 years old, collected at the Laboratory of Medical Psychology FGBI "MHRC" RAMS by its research workers and their students in the period from July 1997 to April 2013 in secondary schools in Moscow City. Based on these data, we obtained normative age-specific development of cognitive processes indicators needed to determine the level of cognitive impairment in patients.

Methods

Pathopsychologic data. Diagnostic data for the three cognitive processes (CP) were obtained using the corresponding pathopsychological tests, each of which was assessed by one parameter:

•   Memory — the amount of immediate verbal memory (10 words);

•   Attention — runtime of Schulte’s test (first table);

•   Thinking — the coefficient of standard thinking (Designing objects test).

Quantitative  assessment  of  cognitive  deficits  in  patients was performed using Z-scales. Z-values were obtained on the basis of normative cognitive processes indicators as the difference of patient score with normative mean referred to the normative standard deviation. Z-values were calculated individually for each patient, taking into account his age up to 1 year: the smaller the resulting Z-value, the greater the deficit. Thus three measures of cognitive deficits were obtained:

•   ZVM — verbal memory deficit volume;

•   ZRS — runtime Schulte’s test deficit;

•   ZST — deficit of the coefficient of standard thinking.

The range of −1 ≤ Z ≤ 1, i.e. ±1 standard deviation from the normative mean, was decided to consider undeficient ("normal"). Z-values below this range were considered deficient, that of above — ahead in cognitive development.

Combinations of deficit indicators in all three evaluated CP were obtained for each patient with varying degrees of deficit intensity (the Z-value) in each of the indicators. Thus it is possible to speak of different degrees of severity of the deficit and types of cognitive development (including deficient) of studied patients.

The degrees of severity of cognitive deficits of each patient were assessed as follows (Figure 1):

•   Significant deficit — the lag from the norm by more than two standard deviations (Z < −2);

•   Moderate deficit — the lag from the norm in the range of one to two standard deviations (−2 < Z < −1);

•   Undeficient state of CP (normative) — not observed deviations from the norm (Z > −1).

Types of cognitive development assessed by "ranking" of the continuum of cognitive development "norm — deficits — defect" based on the ratio of ZVM, ZRS and ZST with age norm: normal (−1,0 ≤ Z ≤ 1,0), behind (Z < −1,0) or ahead (Z > 1,0) it. Five variants of cognitive development were obtained (in three cases this development is a variant of the deficit) outstripping, normal, distorted, deficient and defect. Variants 3, 4 and 5 can be assigned to cognitive deficit according to our ideas about cognitive disontogenesis (see Table 3).

Cluster analysis. To determine the specificity of the deficit of individual cognitive processes and their contribution to the overall picture of the cognitive deficits of patients, we performed a hierarchical cluster analysis by between-group linkage. The analysis was based on ZVM, ZRS and ZCT variables and performed separately for each diagnostic group. Due to the presence of extreme values in ZRS (−32,1; −28,7; −15,9) one patient from the group F21 and two patients from F20.8 were excluded from cluster analysis. As a result the model comprising 3 clusters for each diagnosis has been selected. Further clusters were compared on the following parameters: presence of significant deficit degree in cluster, gender (via the statistic χ²), age and age of onset of patients (using ANOVA and Bonferroni pairwise comparisons) for each diagnosis. Choice of only significant deficit degree to describe clusters was made due to the fact that a moderate degree of deficit can be partly explained by individual subjects’ tests performance, whereas significant deficit is more likely to be due to the disease.

Results

Consider the overall picture of patients’ deficits. Figure 2 shows the mean values of three indicators of deficit for each diagnosis. Attention deficits (ZRS — red in Figure 2) is dominated in all diagnostic groups compared with deficits of memory and thinking and did not differ on average between diagnoses (F(2,204)=0,060; p=0,942). According to our assumption it may be due to the presence of the treatment. Memory deficits (ZVM — blue in Figure 2) was significantly different between all diagnoses (F(2,204)=6,711; p=0,002), most pronounced in the group F20.8 (−1,36), and the lowest in F2x.x (−0,61). Deficit of the coefficient of standard thinking (ZST — green in Figure 2) is most pronounced in the group F20.8 (−1,36), which was significantly lower than that of F21 (−0,81; p=0,014).

Thus, the average attention deficit for F20.8 and F21 reaches pronounced degree, for F2x.x — moderate. Deficits in memory and attention fit to normative rate for the F21 and F2x.x, whereas they are moderate in severity for F20.8.

Types of cognitive development (deficit). Evaluation of patients' cognitive development types presented in Table 4 shows that each diagnosis characterized by a specific pattern of deficit types distribution.

Deficient type of cognitive development is the most common for all patients: 63% of F20.8 group, 65% of F21, and 77% of F2x.x. Outstripping type is the least presented and was not typical for the group F2x.x at all. Differences in the frequency distribution of diagnoses in each type were valid for the norm (p<0,0011), for the deficit (p=0,007) and for the defect types (p<0,001). Defect type represented 24% of patients from the group F20.8, which is significantly greater than in the F21 (p<0,001) and F2x.x groups (p<0,001). Group F21 has the most patients developing normative — 14% (a significant difference with the group F20.8 (p<0,001) and F2x.x (p<0,001)). Distorted type is more common in the group F2x.x compared to F20.8 (p=0,083), and F21 (p=0,048). Deficient type is also more common in F2x.x group than in the other two (significant differences with the group F20.8 (p=0,026) and the F21 (p=0,003)).

In other words, the specific pattern of cognitive impairment is largely determined by the diagnosis: there is "bias" of patients to the defect pole in the group F20.8; the majority of F21 group’s patients tends to the deficient type, and a smaller part of them to the norm type; F2x.x group occupies an intermediate position for account of the most presented deficient and distorted types.

Degree of severity of the deficit was estimated for eight possible combinations of three cognitive processes: deficits of single CP — 1) memory, 2) attention, 3) thinking; combinations of deficits by different CP — 4) memory + attention, 5) memory + thinking, 6) attention + thinking, 7) memory + attention + thinking; 8) no deficits of any CP. Let us consider the data for the significant degree of deficit only. Figure 3 presents the combination described above for the significant deficits in all diagnostic groups. We can see that the majority of patients in each diagnosis characterized either by single significant degree deficits or combinations of them: total is 71% of patients in the F20.8, 59% — in the group F21, and 56% — in the group F2x.x. Attention deficit is the most pronounced among the severe deficits for all diagnoses. Combinations of significant deficits in patients F20.8 together constitute 28,6%, while the remaining two are placed in 15% (12,6% of F21, and 10,3% of F2x.x).

Moderate deficit of thinking has the first place among single moderate deficits; deficit of memory was next in all groups of patients also. Patients from group F20.8 also prevail (20,4%) over the other two groups of patients (12,6% in F21, 10,3% in F2x.x) according to the total number of combinations of deficits of moderate degree. Undeficient development for all three analyzed processes often found in patients with schizotypal disorders (F21) (15%), whereas that of patients in two groups with different types of schizophrenia is only 5,4% (the differences are significant at p≤0,01 by Fisher test).

Consider the data of cluster analysis (CA). Three obtained cluster in each diagnostic group allowed to present a more detailed picture of the specificity of cognitive deficits and a possible link with a number of factors such as gender, age and age of onset. Table 5 shows the means for the variables included in the CA on the basis of which the clusters were formed.

Table 6 shows the values of variables not included in the CA, as well as the general characteristics of the clusters. Table 6 shows that patients unevenly distributed across clusters. The largest clusters were cluster 1 for F20.8, cluster 3 for F21, and cluster 2 for F2x.x. These clusters include patients with the most similar to each other levels of deficits.

From Table 5 and 6 one can see that cluster numbers are not comparable to each other for different diagnoses and are conditional. Consider clusters separately for each diagnosis and factors not included in the CA.

Gender and age. There was not obtained significant differences in the distribution of boys and girls in clusters in any of the diagnostic groups. Significant differences in age between clusters are observed in the group F21 (F(2,115)=10,009; p<0,001) only. Pairwise comparison also shows that the age of the participants in the group F21 in all three clusters differs significantly (p<0,001), with average difference being approximately 1 year (11,1, 12,3 and 13,1 years for 2, 1 and 3 clusters, respectively).

Age of onset. Significant differences were obtained only in the group F20.8 between all clusters on the trend level (F(2,42)=2,996; p=0,061). Pairwise comparisons show the difference between clusters 1 (5,9 years) and 2 (2,5 years) (p=0,056). All F20.8 patients in cluster 2 except for one person became ill in early childhood (from 0 to 4 years) and have pronounced deficits in several cognitive processes.

Discussion

The estimation of specificity of cognitive deficits in children and adolescents with mental disorders is conducted on discrete parameters of cognitive function compared with healthy peers or with older patients. There are rare indications of any specific features of the cognitive profile of patients with schizophrenia spectrum disorders, except for formal thinking disorder, IQ dynamics, various neuropsychological features [2; 9; 11; 17; 18]. Determination of the role of factors contributing to the elaboration of deficit and its features, will allow approaching the complex interaction of cognitive development and disease process — namely, cognitive dysontogenesis.

Using even a small number of parameters of the manifestation of cognitive deficit, one may draw some conclusions about the features of the formation of cognitive functions on the background of schizophrenia spectrum disorders.

Obtained results indicate an important role of clinical factors in the process. Both in adult patients and in children and adolescents the age of onset shows gender differences (see Table 2). Our research showed that boys in average get ill for one or two years earlier than girls and these differences are more obvious in case of childhood schizophrenia (almost 3 years). However, at this stage of the study we could not find an evidence of gender differences in the profile of cognitive deficits. Apparently, we can talk about a different tempo of cognitive development/dysontogenesis among boys and girls, but with close outcome variants. We were also able to confirm the assumption of greater severity of deficit in case of earlier onset of schizophrenia. Most obviously this relationship is seen in 27% of patients belonging to a cluster 2 with childhood schizophrenia in our sample. However, only in the group with this diagnosis the regression analysis of age of onset as the dependent variable from deficit yielded a significant regression model with the inclusion of all three parameters of deficit (R²=0,173; p=0,081), attention and thinking deficits (R²=0,173; p=0,033) and only thinking deficit (R²=0,172; p=0,009). This suggests a direct correlation of early disease onset and the emergence of cognitive decline of, first of all, thinking, in the case of schizophrenia.

Another important clinical factor in determining the specificity of cognitive deficits is nosology. Distinct differences in the manifestation of cognitive deficits among the three diagnostic groups examined patients were observed.

The most severe state of cognitive abilities obtained in patients with childhood type of schizophrenia. Their cognitive development more frequently demonstrates the cognitive dysontogenesis of defect type, which manifests in combination of deficits of different cognitive processes. Moreover, in this group of patients the proportion of those with normal cognitive development is low.

The most safe group are patients with schizotypal disorder which shows high proportion of patients with normal cognitive development, and the main contribution to cognitive deficits is made by problems with attention. For example, the patient who had the greatest attention deficit in the whole sample (ZRS=−32,1) had a diagnosis of F21, while the result in memory and thinking were close to the age-normal ones. Such an interrelationship in deficits raises questions over of environmental influences (primarily treatment) on the state of the cognitive processes that are the most flexible and therefore reactive. Apparently, attention is more determined by the state of the subject while distortions in the more stable cognitive process — thinking — are more shaped by long-term disease process.

Finally, patients with other forms of schizophrenia are situated in a middle position between the other two diagnoses. This group has the highest percentage of distorted type of cognitive development demonstrated in a moderate thinking deficit. Thus it reduces the proportion of normally developing patients. The specialty of this group is also presented in more mature age of the patients (mean 1,25 years they older than the other patients) and a later onset of disease (mean 3,8 years later than other patients). We can assume that the onset of disease occurs after a period of normal cognitive development unaffected by disease, and the current disease process contributes distortion on partially formed cognitive processes.

Conclusions

1.   The elaborated method of assessing of cognitive deficits in children and adolescents with schizophrenia spectrum disorders using a Z-scales highlighted the kinds of cognitive development (from advancing to roughly deficit) occurring in this pathology and set their frequency.

2.   For all groups of patients, the most common type is the deficit of cognitive development with predominant reduction of indicators of cognitive processes compared with the normal development (67%), the most rare — outstripping cognitive development (2%).

3.   Attention indicators are most sensitive for evaluating existing cognitive deficits due to them largely reflecting the state of the patient.

4.   The most severe forms of cognitive deficits occur in childhood type schizophrenia, they have a connection with an earlier age of onset of the disease.
